化积镇痛膏联合腕踝针治疗难治性骨转移癌痛的临床观察

Clinical observation of Huaji Zhentong Ointment combined with wrist-ankle acupuncture for treating refractory bone metastasis pain

目的:评价化积镇痛膏联合腕踝针对难治性骨转移癌痛的镇痛效果。方法:将122例难治性骨转移癌痛患者随机分为骨通贴膏+腕踝针组(30例)、化积镇痛膏+腕踝针组(31例)、骨通贴膏组(29例)、化积镇痛膏组(32例)。比较各组镇痛总有效率、NRS、KPS评分、镇痛起效时间、镇痛时长与不良反应发生率。结果:化积镇痛膏+腕踝针组镇痛总有效率为61.3%,显著高于其他治疗组(P<0.05)。经治疗14d后,化积镇痛膏+腕踝针组NRS评分显著低于其他治疗组,而KPS评分显著高于其他治疗组(P<0.05)。化积镇痛膏+腕踝针组镇痛起效时间为(20.6±13.5)min,持续时间为(8.7±2.3)h,镇痛起效较其他治疗组更快,镇痛持续时间更长(P<0.05)。各组不良反应发生率分别为20.0%、22.6%、20.7%及18.8%,差异无统计学意义。结论:化积镇痛膏联合腕踝针治疗难治性骨转移癌痛疗效可靠、起效迅速、作用持久、安全性好,值得推广。

Objective:To evaluate the analgesic effect of Huaji Zhentong Ointment combined with Wrist-Ankle Acupuncture in the treatment for cancer pain in patients with refractory bone metastases.Methods:A total of 122 patients were randomized into four groups:gu tong tie gao combined with wrist-ankle acupuncture group(30 cases),Huaji Zhentong Ointment combined with wrist-ankle acupuncture group(31 cases),gu tong tie gao group(29 cases),Huaji Zhentong Ointment group(32 cases).The total effective rate of analgesia,number rating scale(NRS)score,Kamofsky Performance Status(KPS)score,the onset time,analgesia maintaining time in each group were compared after treatment.Results:The total effective rate of analgesia was 61.3%in Huaji Zhentong Ointment combined with wrist-ankle acupuncture group which was significantly higher than other groups(P<0.05).After 14 days of treatment,compared with other groups,the NRS score was lower and the KPS score was significantly higher in Huaji Zhentong Ointment combined with wrist-ankle acupuncture group(P<0.05).The onset time was(20.6±13.5)min and the analgesia maintaining time was(8.7±2.3)h in Huaji Zhentong Ointment combined with wrist-ankle acupuncture group,the onset of analgesia is faster and the analgesia maintaining time lasted longer than the other groups(P<0.05).The incidence of adverse reaction in each group were 20.0%,22.6%,20.7%,and 18.8%,respectively,and the difference was not statistically significant.Conclusion:Huaji Zhentong Ointment combined with wrist-ankle acupuncture can increase the efficacy of the pain of patients with bone metastasis cancer,it also can improve the patient’s medication compliance,and more safe.
