液体敷料与中医护理在烙铁头蛇咬伤后局部消肿效果及对炎性因子的影响

Effect of liquid dressing and traditional Chinese medicine nursing on local swelling after venom bite and its effect on inflammatory factors detumescence

目的观察液体敷料与中医护理在烙铁头蛇咬伤后局部消肿效果及对炎性因子的影响。方法将浙江省丽水市中心医院2018年1月—2019年12月接治的60例烙铁头毒蛇咬伤患者作为研究对象,按随机数字表法将其分为两组,均行常规治疗,对照组在常规干预过程中联合中医护理,研究组在对照组患者治疗基础上使用液体敷料,对比两组患者蛇咬伤局部消肿效果、炎性因子变化、术后并发症发生几率。结果干预1、3 d后研究组患者创面湿润、敷料完整性、肿胀程度、创面温度等各项指标均优于对照组,具有统计学意义(P<0.05),研究组患者干预1 d后炎症因子水平低于对照组,具有统计学意义(P<0.05)。研究组患者并发症发生情况与对照组无明显差异P>0.05。结论毒蛇咬伤后采取中医护理可有效降低患者并发症发生率,联合液体敷料后患者炎症因子快速降低,增强局部消肿效果,具有积极的临床使用价值。

Objective To observe the antiinflammatory and repercussive effects of liquid dressing and traditional Chinese medicine nursing in treatment of ironhead snake biting effects of local swelling effect of liquid dressing and traditional Chinese medical nursing after snake(Trimeresurus mucrosquamatus).Methods Sixty patients with ironhead snake bite were randomly divided into 2 equal groups:control group undergoing routine treatment and observation group undergoing TCM nursing with liquid dressing(Sanyrene)in addition of routine treatment.The development of the wound was observed continuously.Peripheral blood samples were collected 1 day after treatment to examine the serum tumor-necrosis factor-α(TNF-α)and、IL-6 curative effects were compared.on the basis of treatment in the control group,and compared the local swelling effect of snake bites,changes in inflammatory factors,and the incidence of postoperative complications in the two groups.Results The indicators of wound moisturization,dressing integrity,swelling degree,and wound temperature of the study group were all better than those of the control group after 1 day and 3 days of intervention(all P<0.05).The levels of TNF-IL-6,and white cells 1 day after intervention of the study group were all significantly lower than those of the control group(all P<0.05).There eas no significant difference in the complication rate between these 2 groups(P>0.05).Conclusion Taking traditional Chinese medicine care after poisonous snake bites can effectively reduce the incidence of complications.Patients′inflammatory factors can be rapidly reduced after combined with liquid dressing,which can enhance the effect of local swelling.It has positive clinical value.
