全腹腔镜远端胃癌根治术对胃癌疗效及患者血清相关指标和预后的影响

Evaluation of the Efficacy of Total Laparoscopic Distal Gastric Cancer Radical Resection on Gastric Cancer and Its Effect on Patients'Serum-related Indicators and Prognosis

目的探讨全腹腔镜远端胃癌根治术对胃癌疗效及患者血清相关指标及预后的影响。方法选取110例胃癌患者作为研究对象,根据手术治疗方式的差异将其分为2组。研究组患者60例,采用全腹腔镜远端胃癌根治术;而对照组患者50例,则采用腹腔镜辅助远端胃癌根治术。比较2组患者的临床疗效等。结果2组患者术中出血量、住院时间及切口长度等差异比较均有统计学意义(P<0.05),而在术后并发症发生率及淋巴结清扫数目方面比较差异无统计学意义(P>0.05)。术前2组患者的相关免疫炎性指标比较无差异(P>0.05),而术后研究组相关免疫炎性指标均比对照组改善明显(P<0.05);术后2组患者的肿瘤相关指标均较术前有所降低(P<0.05),但组间比较无差异(P>0.05)。术后1年内,2组患者的生存率比较均无差异(P>0.05)。结论全腹腔镜远端胃癌根治术治疗胃癌的疗效较佳,同时对患者的炎性反应较轻,且不影响患者预后,值得临床推广应用。

Objective To investigate the effect of total laparoscopic distal gastric cancer radical resection on gastric cancer and its effect on patients'serum-related indicators and prognosis.Methods 110 patients with gastric cancer were selected,and according to the difference of the surgical treatment,were divided into 2 groups.60 patients in the study group were treated with laparoscopic radical gastrectomy,while 50 patients in the control group were treated with laparoscopic assisted distal gastric cancer root treatment,and the clinical efficacy of the two groups were compared.Results There were significant differences in operation time,intraoperative blood loss,postoperative pain score,length of hospital stay and incision length between the 2 groups and the control group(P<0.05),but there were no differences in the number of lymph node dissection and the incidence of postoperative complications between the 2 groups(P>0.05);There was no difference in the related immune inflammatory indexes between the 2 groups before operation(P>0.05),but the related immune inflammatory indexes in the study group were significantly improved after operation(P<0.05);The tumor related indexes of the 2 groups after operation were lower than those before operation(P<0.05),but there was no difference between the 2 groups(P>0.05);Within one year after operation,there was no difference in the survival rate between the 2 groups(P>0.05).Conclusion Total laparoscopic distal gastrectomy has better curative effect on gastric cancer patients,and has less inflammatory reaction to the body,and does not affect the prognosis of patients,so it is worthy of clinical application.
