结肠镜联合血清肿瘤标志物鉴别结直肠良恶性息肉的价值

Value of Colonoscopy Combined with Serum Tumor Markers in Differentiating Colorectal Benign Polyps and Malignant Carcinoma

目的研究结肠镜联合血清肿瘤标志物鉴别结直肠良恶性息肉的价值。方法选取62例结直肠息肉患者,经病理检查确诊,良性35例、恶性27例。对患者进行结肠镜检查、血清CEA、CA125等肿瘤标志物检测;以病理诊断结果为金标准,比较结肠镜、血清CEA、CA125水平对结直肠良恶性息肉诊断结果,分析结肠镜与血清CEA、CA125单独与联合检测的灵敏性与特异性。结果结肠镜检查的灵敏性为74.29%(26/35)、特异性为85.19%(23/27)、准确度为70.03%(49/62)。血清CEA联合CA125检测的灵敏性为80.00%(28/35)、特异性为81.48%(22/27)、准确度为80.65%(50/62)。结肠镜与血清CEA、CA125联合检测的准确度、灵敏性与特异性显著高于其单独检测(P<0.05)。结论结肠镜联合血清CEA、CA125等肿瘤标志物检测可提高对结直肠良恶性息肉的诊断效能,为临床早期判断结直肠病变程度提供参考,推荐使用。

Objective To investigate the value of colonoscopy combined with serum tumor markers in differentiating colorectal benign polyps and malignant carcinoma.Methods A total of 62 patients with colorectal polyps were selected and confirmed by pathological examination,including 35 cases of benign and 27 cases of malignant.All patients received colonoscopy,serum CEA,CA125 and other tumor marker tests.Taking pathological examination as golden standard,the value of colonoscopy,serum CEA and CA125 in differentiating benign and malignant colorectal polyps were compared;Then the sensitivity and specificity of combined and single examination of colonoscopy,serum CEA and CA125 were analyzed.Results The sensitivity,specificity and accuracy of colonoscopy were 74.29%(26/35),85.19%(23/27)and 70.03%(49/62).The sensitivity,specificity and accuracy of serum CEA combined with CA125 were 80.00%(28/35),81.48%(22/27)and 80.65%(50/62).The accuracy,sensitivity and specificity of colonoscopy combined with serum CEA and CA125 were significantly higher than those of single detection(P<0.05).Conclusion Colonoscopy combined with serum CEA,CA125 and other tumor marker tests can effectively improve the diagnostic efficacy of benign and malignant colorectal polyps,and provide reference for early clinical evaluation,so it is worthy of recommendation.
