22q12.1-q12.2微缺失相关神经纤维瘤病Ⅱ型患者经迷路入路同期肿瘤切除并人工耳蜗植入一例 被引量:1

Simultaneous cochlear implantation and translabyrinthine removal of vestibular schwannoma in type 2 neurofibromatosis caused by a deletion of 22q12.1-q12.2 including NF2 gene

摘要 报道1例22q12.1-q12.2微缺失相关神经纤维瘤病Ⅱ型(NF-2)患者经迷路入路同期肿瘤切除并人工耳蜗植入的患者。患者男,40岁,汉族,主因双耳渐进性听力下降8年余入院。纯音测听示右耳全聋,左耳重度聋;言语识别率右耳0,左耳88%;听性脑干反应(ABR)右耳最大给声强度未引出反应,左耳波形分化差,Ⅰ~Ⅴ间期延长;畸变产物耳声发射双耳各频率均未引出反应。前庭功能检查提示右侧前庭功能明显减低。颞骨CT检查提示双侧内听道轻度扩大;MR检查提示双侧桥小脑角区见结节状稍长T1稍长T2信号,增强扫描双侧病变均呈结节状明显强化。基因检测发现第22号染色体长臂部分22q12.1-q12.2片段缺失1个拷贝,长约2.3385 Mb,该区域覆盖了NF-2的致病基因NF2基因。该患者不伴有其他颅神经症状,无眼部及皮肤病变,诊断为NF-2,临床分型为Gardner型。完善检查后于全身麻醉下经迷路进路行右侧听神经瘤切除术,同期右侧经圆窗人工耳蜗植入术。术后10个月复查,听力评估提示右耳有较好的助听听阈。术后18个月随访,患者诉耳蜗效果较前明显提高,听觉能力分级(CAP)为6级,言语可懂度分级(SIR)为4级。
