微型钢板联合Nice结与单微型钢板内固定治疗掌指骨斜形骨折的对比研究

Comparison of micro plate combined with Nice knot and single micro plate in the treatment of oblique metacarpal and phalangeal fractures

目的比较微型钢板联合Nice结内固定与单微型钢板内固定治疗掌指骨斜形骨折的临床疗效探讨。方法选取63例掌指骨斜形骨折并行手术治疗患者,按照随机数字表法分为两组,对照组使用单微型钢板内固定治疗,研究组使用微型钢板联合Nice结内固定治疗。比较两组患者手术时间、术后24 h疼痛视觉模拟评分(VAS)、骨折愈合时间、术后患指活动优良率及并发症发生率。结果研究组在术后24 h疼痛VAS评分、术后患指活动优良率及并发症发生率均优于对照组,差异均有统计学意义(t=9.88,χ^(2)分别=5.67、3.95,P均<0.05),两组患者手术时间和骨折愈合时间比较,差异均无统计学意义(t分别=1.07、1.21,P均>0.05)。结论微型钢板联合Nice结内固定治疗掌指骨斜形骨折,操作简单,软组织刺激小,能有效保护骨折端周围血运,并可提供坚强内固定,是一种良好的手术方法。

Objective To compare the clinical effect of micro plate combined with Nice knot and single micro plate in the treatment of oblique metacarpal and phalangeal fractures.Methods A total of 63 patients with oblique metacarpophalangeal fractures were divided into two groups according to the random numbers.The control group was treated with single micro plate internal fixation,and the study group was treated with micro plate combined with Nice knot internal fixation.The operation time,24 h postoperative visual analogue scale(VAS),fracture healing time,excellent and good rate of postoperative finger movement and the incidence of complications were compared between the two groups.Results The VAS score,the excellent and good rate of postoperative finger movement and the incidence of complications in the study group were better than those in the control group(t=9.88,χ^(2)=5.67,3.95,P<0.05).There were no significant differences in operation time and fracture healing time between the two groups(t=1.07,1.21,P>0.05).Conclusions Micro plate combined with Nice knot internal fixation for the treatment of oblique metacarpophalangeal fractures has the advantages of simple operation and small soft tissue stimulation.It can effectively protect the blood supply around the fracture end and provide strong internal fixation.It is a good surgical method.
