不同辅助治疗方案对宫腔粘连患者宫腔粘连分离术后妊娠的影响

Effect of different adjuvant therapy on Pregnancy after separation of Intrauterine adhesions

目的探讨不同辅助治疗方案对宫腔粘连患者宫腔粘连分离术后妊娠的影响。方法选取100例行宫腔粘连分离术治疗的宫腔粘连患者,按照随机数字表法分为对照组与试验组,每组50例。对照组采用西药治疗,试验组在对照组基础上采用中药治疗。比较两组宫腔粘连程度、子宫容受性量化指标、月经恢复情况、妊娠成功率及生活质量评分。结果试验组宫腔粘连程度明显轻于对照组,差异有统计学意义(P<0.05)。试验组子宫内膜厚度明显厚于对照组,子宫动脉、螺旋动脉搏动指数(PI)、阻力指数(RI)指标均明显低于对照组,宫腔容积明显大于对照组,差异有统计学意义(P<0.05)。试验组月经恢复总有效率、妊娠成功率分别为98.00%、96.00%,均明显高于对照组的82.00%、78.00%,差异有统计学意义(P<0.05)。治疗前,两组生理功能、生理职能、躯体疼痛、总体健康、社会功能、精力、情感职能、精神健康评分比较,差异无统计学意义(P>0.05);治疗后,试验组明显高于对照组,差异有统计学意义(P<0.05)。结论中药联合西药辅助治疗,可提高宫腔粘连分离术患者妊娠率,提升生活质量,值得临床推广与应用。

Objective To investigate the effect of different adjuvant therapy on pregnancy after separation of uterine adhesions.Methods 100 patients with intrauterine adhesions treated by intrauterine adhesions separation were selected and divided into control group and experimental group according to random number table,50 cases in each group.The control group was treated with western medicine,and the experimental group was treated with Traditional Chinese medicine on the basis of the control group.The degree of intrauterine adhesion,quantitative indexes of uterine receptivity,menstrual recovery,pregnancy success rate and quality of life score of the two groups were compared.Results The degree of intrauterine adhesion in experimental group was significantly lighter than that in control group,and the difference was statistically significant(P<0.05).The endometrial thickness of experimental group was significantly thicker than that of control group,the indexes of uterine artery,spiral artery pulse index(PI)and resistance index(RI)were significantly lower than that of control group,and the uterine cavity volume was significantly higher than that of control group,with statistical significance(P<0.05).The total effective rate of menstrual recovery and pregnancy success rate of experimental group were 98.00%and 96.00%,respectively,which were significantly higher than 82.00%and 78.00%of the control group,with statistical significance(P<0.05).Before treatment,there were no significant differences in physiological function,physical function,physical pain,general health,social function,energy,emotional function and mental health scores between the two groups(P>0.05).After treatment,the experimental group was significantly higher than the control group,the difference was statistically significant(P<0.05).Conclusion TCM combined with western medicine can improve pregnancy rate and quality of life in patients with intrauterine adhesions separation,which is worthy of clinical promotion and application.
