摘要
目的探讨居家应用可穿戴单导联远程心电监测设备采集1 h和24 h数据在心律失常筛查中的效果。方法选取1000例患者有心悸、胸闷、气短等症状时居家应用可穿戴单导联远程心电监测设备采集的500份24 h心电图(24 h组)和500份1 h心电图(1 h组)资料,其中既往有心血管病史者441例(有病史组),无心血管病史者559例(无病史组)。统计心律失常检出率及检出类型。结果24 h组心律失常检出率高于1 h组(P<0.05),其中有病史组和无病史组24 h心电图心律失常检出率均高于1 h心电图(P<0.05)。24 h组和1 h组分别检出14种类型653例次、9种类型348例次心律失常,其中有病史组24 h和1 h心电图分别检出13种类型333例次、7种类型180例次心律失常;无病史组24 h和1 h心电图分别检出11种类型320例次、6种类型168例次心律失常。结论居家应用可穿戴单导联远程心电监测设备采集24 h数据不但可提高心律失常检出率及检出类型,协助解决紧急医疗事件,还能保障患者的依从性,降低检查费用。
Objective To explore the effect of home-use wearable single-lead remote ECG monitoring equipment for collecting 1-hour and 24-hour data in arrhythmia screening.Methods 500 patients with 24-hour ECG(24-hour group)and 500 patients with 1-hour ECG(1-hour group)were collected from 1000 patients with palpitation,chest tightness,shortness of breath and other symptoms at home using wearable single lead remote ECG monitoring equipment.Out of 1000 patients,441 patients had a history of cardiovascular disease,and 559 patients had no history of cardiovascular disease.The detection rate and types of arrhythmia were analyzed.Results The detection rate of arrhythmia in 24-hour group was higher than that in 1-hour group(81.8%vs.54.6%,P<0.05);The detection rate of arrhythmia in patients with history of cardiovascular disease(history group)in 24-hour ECG was higher than that in patients with not history of cardiovascular disease(no history group)in 1-hour ECG(89.5%vs.59.3%,P<0.05);There were 653 arrhythmias of 14 types and 348 arrhythmias of 9 types detected in 24-hour group and 1-hour group respectively.Among them,333 arrhythmias of 13 types and 180 arrhythmias of 7 types were detected in 24-hour and 1-hour ECG of patients with or with not history cardiovascular disease respectively;The 24-hour and 1-hour electrocardiograms in the no history group detected 320 cases of 11 types and 168 cases of 6 types of arrhythmias respectively.Conclusion The home-use wearable single-lead remote ECG monitoring equipment to collect 24-hour data can not only improve the detection rate and type of arrhythmia that can help to solve the emergency medical events,but also ensure the compliance of patients and reduce the cost of examination.
