剖宫产围术期实施加速康复外科措施对产妇术后临床指标的影响

Effect of ERAS on Postoperative Clinical Indexes during Perioperative Period of Cesarean Section

目的探讨在剖宫产围术期实施加速康复外科措施的效果。方法选择2019年1月—2019年6月在福建省妇幼保健院行择期剖宫产产妇,根据围术期处理方法的不同将其分为观察组82例及对照组124例,观察组在围术期实施加速康复外科措施,对照组在围术期实施常规措施,对比两组的术后相关指标及产妇和新生儿情况。结果两组产妇术中出血量及新生儿体质量差异均无统计学意义(P值均>0.05)。观察组产妇术后住院时间及术后开始排气时间和自主活动时间均短于对照组,住院费用(在剔除特需病房收费及单病种原因后)少于对照组,两组上述指标差异均具有统计学意义(P值均<0.05),两组上述指标经多重线性回归校正混杂因素后差异均具有统计学意义(P值均<0.05)。结论在行剖宫产产妇的围术期实施加速康复外科措施,有助于促进产妇的术后恢复,减少住院费用,具有一定的推广价值。

Objective To explore the effect of enhanced recovery after surgery(ERAS)concept and measures on the postoperative clinical indexes after cesarean section.Methods The patients who received elective cesarean section in Fujian Maternity and Child Health Hospital from January 2019 to June 2019 were divided into observation group(82 cases)and control group(124 cases)according to different perioperative treatment.The observation group adopted ERAS during perioperative period,and the control group adopted routine measure.Postoperative indicators and maternal and neonatal situation were compared between the two groups.Results There was no significant difference in postpartum blood loss and neonatal weight between the two groups(P>0.05).The duration of hospitalization,exhaust time and postoperative autonomic activity in the observation group were significantly shorter than those in the control group,and the hospitalization cost in the observation group was significantly lower than that in the control group(P<0.05).These differences remained significant after multiple linear regressions adjusted for confounders(P<0.05).Conclusion The integration ERAS in clinical medical work is helpful to accelerate the recovery after cesarean section and reduce the clinical costs,and the promotion of ERAS concept during the perioperative period of cesarean section has important application significance.
