腹腔镜下Miles术和Dixon术治疗老年低位直肠癌的效果比较

Comparison of laparoscopic Miles operation and Dixon operation in the treatment of elderly low rectal cancer

目的比较腹腔镜下Miles术和Dixon术治疗老年低位直肠癌的效果。方法收集我院2017年1月至2020年5月收治的53例老年低位直肠癌患者作为研究对象,根据治疗方法不同将其分为观察组(n=28)和对照组(n=25)。对照组给予腹腔镜下Miles术,观察组给予腹腔镜下Dixon术。比较两组的临床治疗效果。结果观察组的手术时间、术后排气时间、住院时间短于对照组,术中出血量少于对照组(P<0.05)。术后6个月、术后1年,两组的PAC-QOL评分均较术前升高,但观察组显著低于对照组(P<0.05)。术后1年,观察组的排尿功能优于对照组(P<0.05)。两组的术后并发症总发生率,术后1年的复发率、远处转移率和生存率无显著差异(P>0.05)。结论腹腔镜下Dixon术治疗老年低位直肠癌有助于早期恢复,对排尿功能影响小,可改善患者的生活质量,值得推广。

Objective To compare the effects of laparoscopic Miles operation and Dixon operation in the treatment of elderly low rectal cancer.Methods A total of 53 elderly patients with low rectal cancer treated in our hospital from January 2017 to May 2020 were collected as research objects.According to different treatment methods,the patients were divided into observation group(n=28)and control group(n=25).The control group was given laparoscopic Miles operation,and the observation group was given laparoscopic Dixon operation.The clinical therapeutic effects of the two groups were compared.Results The operation time,postoperative exhaust time and hospital stay in the observation group were shorter than those in the control group,and the amount of intraoperative bleeding was less than that in the control group(P<0.05).At 6 months and 1 year after operation,the PAC-QOL scores of the two groups were higher than those before operation,but those of the observation group were significantly lower than the control group(P<0.05).One year after operation,the urination function of the observation group was better than that of the control group(P<0.05).There were no significant differences in the total incidence of postoperative complications,recurrence rate,distance metastasis rate and survival rate at 1 year after operation between the two groups(P>0.05).Conclusion Laparoscopic Dixon operation in the treatment of elderly patients with low rectal cancer is conducive to early recovery,which has little impact on urination function,and can improve the quality of life of patients,which is worthy of promotion.
