基于关联规则的31958张含麻黄中药制剂处方用药分析

Analysis on 31958 TCM Prescriptions Containing Ephedrae Herba Based on Association Rules

目的分析首都医科大学附属北京中医医院(以下简称“本院”)门诊含麻黄中药制剂的使用情况,总结用药风险,以期为含麻黄中药制剂的临床合理使用、降低用药风险提供参考。方法通过本院医院信息管理系统调取2019年1月1日-6月30日门诊使用含麻黄中药制剂处方31958张,采用关联规则分析方法,从“药品-药品”“药品-诊断”关联角度分析用药情况。结果本院使用含麻黄中药制剂共17种,使用此类药品的患者以中年组(36~60岁)为主;开具处方最多的科室为呼吸科门诊;由“药品-药品”关联分析可知,存在多种含麻黄中药制剂联用情况,联用2种及以上的处方约30%,不宜与含麻黄中药制剂联用的西药处方约15%,此类西药存在联用2~4种含麻黄中药制剂的情况;由“诊断-药品”关联分析可知,约12%特殊疾病患者存在使用含麻黄中药制剂的情况,使用药品中存在2~4种含麻黄中药制剂的情况。结论含麻黄中药制剂的使用存在用药风险,应加强麻黄相关药品的不良反应监测,探索建立含麻黄中药制剂的相关使用规范,保障患者用药安全。

Objective To analyze the use of TCM preparations containing Ephedrae Herba in outpatient of Beijing Hospital of Traditional Chinese Medicine Affiliated to Capital Medical University(hereinafter referred to as“our hospital”);To summarize the situation of drug use risk;To provide a reference for the rational clinical use of TCM preparations containing Ephedrae Herba and reduce the risk of medication.Methods Through the hospital information management system of our hospital,totally 31958 TCM prescriptions containing Ephedrae Herba from January 1st to June 30th,2019 were retrieved.The association rule analysis method was used to analyze the drug use from the perspective of“drug-drug”and“drug-diagnosis”association.Results There were 17 kinds of TCM prescriptions containing Ephedrae Herba in our hospital,most of which were issued by middle-aged people(36 to 60 years old),and were most commonly used in respiratory clinics.According to the“drug-drug”correlation analysis,there were many combinations of TCM prescriptions containing Ephedrae Herba,and the combination of two or more medicines accounted for about 30%.The proportion of prescriptions of Western medicines should not be combined with TCM prescriptions containing Ephedrae Herba was about 15%.Moreover,two to four kinds of TCM prescriptions containing Ephedrae Herba were also used in those kinds of people who used Western medicines at same times.According to the“diagnosis-drug”correlation analysis,about 12%of patients with special diseases used TCM prescriptions containing Ephedrae Herba,and there were 2 to 4 kinds of TCM prescriptions containing Ephedrae Herba in the medicines used.Conclusion There were risks in the use of TCM prescriptions containing Ephedrae Herba.It is recommended to strengthen the monitoring of adverse reactions of Ephedrae Herba related drugs,and explore the establishment of relevant usage standards of TCM preseriptions containing Ephedrae Herba,so as to ensure the medication safety of patients.
