维生素D缺乏和基因多态性与自身免疫性甲状腺疾病相关性研究进展 被引量:7

摘要 维生素D不仅能够调节钙磷代谢、保持骨骼健康,而且对肿瘤、自身免疫性疾病、糖尿病和心血管疾病等均有重要影响,在免疫调节、细胞增殖和抗炎等方面发挥重要作用。维生素D发挥生物学作用主要通过1,25-二羟维生素D_(3)与维生素D受体(VDR)结合,通过调节免疫细胞分化成熟及细胞因子表达,进而影响自身免疫性甲状腺疾病(AITD)的发生发展。此外,VDR基因多态性与AITD的遗传易感性显著相关,具体机制尚未阐明,可能由于VDR基因多态性导致VDR结构异常,以及维生素D缺乏通过免疫机制、激素水平及自身抗体等影响AITD的演变转归。因此,未来深入探讨维生素D缺乏与AITD的相关机制,对指导AITD的预防及诊疗具有重要意义。 Vitamin D not only can regulate calcium and phosphorus metabolism and maintain bone health,but also has significant effect on tumor,autoimmune diseases,diabetes and cardiovascular diseases,playing an important role in immune regulation,cell proliferation and anti-inflammation.Vitamin D plays a biological function mainly through the combination of 1,25-dihydroxy vitamin D_(3) with vitamin D receptor(VDR),through regulating the differentiation and maturation of immune cells and the expression of cytokines,thus affecting the occurrence and deve-lopment of autoimmune thyroid diseases(AITD).In addition,VDR gene polymorphism is significantly associated with genetic susceptibility to AITD,but the specific mechanism has not been elucidated.It is possible that VDR gene polymorphism leads to structural abnormalities in VDR,and vitamin D deficiency affects the evolution and outcome of AITD through immune mechanisms,hormone levels and autoantibodies.Therefore,further in-depth research on the mechanisms of vitamin D deficiency and AITD is of great significance for the prevention,diagnosis and treatment of AITD.
