3D-TVUD技术在不孕女性IVF-ET后妊娠结局评估中的应用价值 被引量:2

摘要 目的分析经阴道三维超声多普勒(3D-TVUD)技术在不孕女性体外受精-胚胎移植(IVF-ET)后妊娠结局评估中的应用价值。方法选择2020年1—12月湘潭市中心医院接受IVF-ET后妊娠失败的40例不孕女性患者作为观察组,同期纳入接受IVF-ET后成功妊娠的40例不孕女性患者作为对照组,两组患者均于胚胎移植当天接受3D-TVUD技术检查,比较两组子宫内膜厚度、内膜容积、内膜形态类型和内膜容积血流参数,分析3D-TVUD技术在不孕女性IVF-ET后妊娠结局中的评估价值。结果观察组收缩峰速度与舒张末期速度比值(S/D)、搏动指数(PI)、阻力指数(RI)均高于对照组[2.83(2.62,3.16)比2.58(2.40,2.79)、1.43(1.27,1.56)比1.26(1.08,1.38)、(0.65±0.07)比(0.57±0.09)],且血管化指数(VI)、血流指数(FI)及血管化血流指数(VFI)均低于对照组[5.90(5.23,6.75)比6.84(6.26,7.94)、21.02(20.55,22.39)比22.78(21.33,24.40)、(2.92±0.69)比(3.56±0.66)](P<0.01);两组子宫内膜厚度、容积及内膜形态类型比较差异无统计学意义(P>0.05)。绘制受试者工作特征曲线,S/D、PI、RI、VI、FI、VFI评估孕女性IVF-ET后妊娠结局的曲线下面积分别为0.767、0.747、0.760、0.777、0.783、0.739,评估价值较理想。结论3D-TVUD技术在不孕女性IVF-ET后妊娠结局评估中的应用价值较高,可用于早期评估不孕女性IVF-ET后妊娠结局情况。 Objective To analyze the application value of three-dimensional transvaginal ultrasound Doppler(3D-TVUD)technology in the evaluation of pregnancy outcome of infertile women after in vitro fertilization and embryo transfer(IVF-ET).Methods A total of 40 infertile women with pregnancy failure after IVF-ET in Xiangtan Central Hospital from Jan.2020 to Dec.2020 were included as an observation group,and 40 infertile women with successful pregnancy after IVF-ET in the hospital during the same period were included as a control group.Both groups received 3D-TVUD technology on the day of embryo transfer,and endometrial thickness,endometrial volume,endometrial morphology and endometrial volume blood flow parameters of the two groups were compared,and the value of 3D-TVUD technology in the assessment of pregnancy outcome of infertile women after IVF-ET was analyzed.Results The ratio of peak systolic velocity/end diastolic velocity(S/D),pulsatility index(PI)and resistance index(RI)in the observation group were higher than those in the control group[2.83(2.62,3.16)vs 2.58(2.40,2.79),1.43(1.27,1.56)vs 1.26(1.08,1.38),(0.65±0.07)vs(0.57±0.09)],vascularization index(VI),flow index(FI)and vascularization-flow index(VFI)were lower than those of the control group[5.90(5.23,6.75)vs 6.84(6.26,7.94),21.02(20.55,22.39)vs 22.78(21.33,24.40),(2.92±0.69)vs(3.56±0.66)](P<0.01).There was no statistical significant difference in endometrial thickness,volume and endometrial morphology between the two groups(P>0.05).The receiver operating curve was drawn,the area under the curve of S/D,PI,RI,VI,FI and VFI in evaluating pregnancy outcome of infertile women after IVF-ET were 0.767,0.747,0.760,0.777,0.783 and 0.739 respectively,which had ideal value for the assessment.Conclusion 3D-TVUD technology has high applicative value in the assessment of pregnancy outcome of infertile women after IVF-ET,which can be used for the early assessment of pregnancy outcome of infertile women after IVF-ET.
