乳腺癌彩色多普勒血流成像与雌激素受体、人表皮生长因子受体2、p53和Ki-67的相关性分析 被引量:5

摘要 目的分析乳腺癌彩色多普勒血流成像(CDFI)特征与雌激素受体(ER)、人表皮生长因子受体2(HER2)、p53及Ki-67的相关性。方法分析76例乳腺癌患者的CDFI特征(肿瘤直径、边缘毛刺征、微钙化、血流分级、淋巴结转移、后方回声衰减),并采用Spearman法分析乳腺癌CDFI特征与ER、HER2、p53及Ki-67的相关性。结果乳腺癌患者乳腺癌组织中ER、HER2、p53及Ki-67阳性表达率分别为73.68%(56/76)、60.53%(46/76)、48.68%(37/76)、40.79%(31/76),其中ER阳性表达率最高,Ki-67阳性表达率最低。有边缘毛刺征、血流分级为0~1级、有淋巴结转移的乳腺癌患者乳腺癌组织中ER阳性表达率均高于无边缘毛刺征、血流分级为2~3级、无淋巴结转移者(P﹤0.05)。有微钙化、血流分级为2~3级、有淋巴结转移的乳腺癌患者乳腺癌组织中HER2阳性表达率均高于无微钙化、血流分级为0~1级、无淋巴结转移者(P﹤0.05)。血流分级为2~3级、有淋巴结转移的乳腺癌患者乳腺癌组织中p53及Ki-67阳性表达率均高于血流分级为0~1级、无淋巴结转移者(P﹤0.05)。乳腺癌患者乳腺癌组织中ER阳性表达率与肿瘤边缘毛刺征、淋巴结转移均呈正相关(P﹤0.05),与血流分级呈负相关(P﹤0.05),而与肿瘤直径、微钙化、后方回声衰减均无相关性(P﹥0.05)。乳腺癌患者乳腺癌组织中HER2阳性表达率与肿瘤微钙化、血流分级、淋巴结转移均呈正相关(P﹤0.05),而与肿瘤直径、边缘毛刺征、后方回声衰减均无相关性(P﹥0.05)。乳腺癌患者乳腺癌组织中p53、Ki-67阳性表达率与血流分级、淋巴结转移均呈正相关(P﹤0.05),而与肿瘤直径边缘毛刺征、微钙化、后方回声衰减均无相关性(P﹥0.05)。结论乳腺癌患者CDFI特征与ER、HER2、p53及Ki-67阳性表达存在一定的相关性,在预测乳腺癌生物学特性中具有一定的指导价值。 Objective To analyze the correlation between color Doppler flow imaging(CDFI)of breast cancer with the levels of estrogen receptor(ER),human epidermal growth factor receptor-2(HER2),anti-oncogene p53 and Ki-67.Method The CDFI features(tumor diameter,marginal burr,microcalcification,blood flow classification,lymph node metastasis,and posterior attenuation)were analyzed in 76 patients with breast cancer.The correlation between CDFI characteristics of breast cancer with ER,HER2,p53 and Ki-67 were analyzed by Spearman.Result The positive expression rates of ER,HER2,p53 and Ki-67 in patients with breast cancer were 73.68%(56/76),60.53%(46/76),48.68%(37/76)and 40.79%(31/76),respectively.Among them,the positive expression rate of ER was the highest,and positive expression rate of Ki-67 was the lowest.The positive rate of ER expression in breast cancer patients with marginal burrs,blood flow classification 0-1 grade and lymph node metastasis were higher than those without marginal burrs,blood flow classification 2-3 grade and lymph node metastasis(P<0.05).The positive rate of HER2 expression in breast cancer patients with microcalcification,blood flow classification 2-3 grade and lymph node metastasis were higher than those without microcalcification,blood flow classification 0-1 grade and lymph node metastasis(P<0.05).The positive expression rates of p53 and Ki-67 in breast cancer patients with blood flow classification 2-3 grade and lymph node metastasis were higher than those with blood flow classification 0-1 grade and without lymph node metastasis(P<0.05).The positive expression rate of ER in breast cancer tissues was positively correlated with marginal burr and lymph node metastasis(P<0.05),and negatively correlated with blood flow grading(P<0.05),but not correlated with tumor diameter,microcalcification and posterior attenuation(P>0.05).The positive expression rate of HER2 in breast cancer tissues was positively correlated with tumor microcalcification,blood flow grading and lymph node metastasis(P<0.05),but not correlated with tumor diameter,marginal burr sign and posterior attenuation(P>0.05).The positive expression rates of p53 and Ki-67 in breast cancer tissues were positively correlated with blood flow grade and lymph node metastasis(P<0.05),but not correlated with tumor diameter,marginal burr sign,microcalcification and posterior attenuation(P>0.05).Conclusion There is correlation between the CDFI signs of patients with breast cancer to some extent and the positive expression of ER,HER2,p53 and Ki-67,which has guiding value in predicting the biological characteristics of breast cancer to some extent.
