运用系统思维建立新冠肺炎医院感染“大感控”策略研究

A Study on Establishing the Strategy of“Macro Infection Prevention and Control”of Nosocomial COVID-19 Infection From the Perspective of System Theory

采用回顾性兼前瞻性研究方法,对国内、外新冠肺炎流行趋势、防控举措及效果进行了分析,重点解决近两年来新冠肺炎医院感染控制出现的漏洞和短板,提出医院感染“大感控”新理念,运用系统思维对构建新冠肺炎医院感染“大感控”策略进行了研究。提出将医院感染控制工作预案列入政府应急“大预案”范畴;建立医院感染控制政府和社会支持保障体系;对区域性医疗资源进行整合与实施分类管理;建立定点(后备)医院感染控制能力评价指标等四项系统控制举措,为政府决策,实现医院感染科学防控、精准防控和合理配置资源提供技术支持。

This paper conducts an analysis of epidemic trend,prevention and control measures and effects of COVID-19 by adopting retrospective and forward-looking study methods,focuses on solving loopholes and weak⁃nesses appearing in nosocomial COVID-19 infection prevention and control over the past two years,puts forward the new concept of“macro infection prevention and control”against nosocomial infection,and uses system theory to conduct a study of establishing the strategy of“macro infection prevention and control”of nosocomial COVID19 infection.It puts forward the four specific measures of incorporating the emergency plan for nosocomial infection and control into“macro emergency plan”of government,establishing governmental and social support security system for nosocomial infection control,integrating and conducting classified management of regional medical re⁃sources,and establishing evaluation indicators for infection control capacity of fixed-point(reserve)hospital with a view to providing technical support for government policymaking on scientific and precision prevention and control of nosocomial COVID-19 infection,and rational resource allocation.
