基于跨理论模型为框架的动机性访谈对维持血液透析患者遵医行为的影响

The effect of motivational interview pair maintenance based on cross-theoretical model on of compliance behavior of hemodialysis patients

目的探讨基于跨理论模型(TTM)为框架的的动机性访谈对维持血液透析患者遵医行为的影响。方法选取2019年8月—2020年8月医院收治的维持血液透析患者150例,按照组间基本特征匹配的原则分为观察组与对照组,每组75例。对照组患者血液透析期间行常规健康指导,观察组实施基于TTM为框架的的动机性访谈,比较两组患者干预前后遵医行为、并发症发生率及GQOL-74生活质量综合评定问卷评分。结果干预后,观察组患者遵医行为总评分及GQOL-74生活质量综合评定问卷评分高于对照组(P<0.05);抽筋、低血压、钙磷代谢紊乱、营养不良等并发症发生率低于对照组(P<0.05)。结论基于TTM为框架的的动机性访谈能有效提高维持血液透析患者遵医行为,降低患者血液透析相关并发症发生率,提高其生活质量。

Objective To explore the eff ect of motivational interviewing based on cross-theoretical model(TTM)on maintaining hemodialysis patients'compliance behavior.Methods A total of 150 patients with maintenance hemodialysis admitted to hospital from August 2019 to August 2020 were selected and divided into observation group and control group according to the principle of basic features matching between groups,with 75 cases in each group.The control group received routine health guidance during hemodialysis,the observation group received motivational interview based on TTM framework.The compliance behavior,incidence of complications and score of GQOL-74 comprehensive assessment questionnaire before and after intervention were compared between the two groups.Results After intervention,the total score of compliance behavior and GQOL-74 quality of life questionnaire in observation group were higher than the control group(P<0.05).The incidence of complications such as cramps,hypotension,calcium and phosphorus metabolism disorder and malnutrition were lower than the control group(P<0.05).Conclusion Motivational interview based on TTM framework could effectively improve the maintenance of hemodialysis patients'compliance behavior,reduce the incidence of hemodialysis-related complications and improve their quality of life.
