摘要
目的探究高原红细胞增多症患者肠道菌群变化及相关意义。方法招募西藏那曲地区血红蛋白(hemoglobin,HB)含量高于210g/L的高原红细胞增多症患者12人,对照组(170g/L<HB<210g/L)42人,采集粪便、血液等样本。从中选取高原红细胞增多症患者9人,对照组16人粪便样本提取DNA,进行16SrRNA测序分析。结果高原红细胞增多症患者的血红蛋白水平以及红细胞比容显著高于对照组。红细胞增多症患者的肠道菌群整体结构改变不明显,多样性没有显著差异。红细胞增多症患者的粪便菌群出现厚壁菌门中梭状芽胞杆菌BB60属、纺锤链杆菌属、厌氧棒杆菌属、霍氏真杆菌,变形菌门中假单胞菌属,拟杆菌门中副拟杆菌属丰度的显著增高。这些菌属改变经KEGG预测分析显示与代谢紊乱相关。斯皮尔曼分析显示多种菌群改变与血红蛋白水平以及红细胞比容正相关。结论高原红细胞增多症患者肠道菌群出现紊乱,菌群变化与代谢紊乱和红细胞增多相关。
Objective To investigate the changes of gut microbiota in patients with plateau erythrocytosis and the related significance.Methods A total of 12high altitude Polycythemia patients with hemoglobin(HB)above 210g/L and 42subjects in the control group(170g/L<HB<210g/L)were recruited in Naqu,Tibet.Stool and blood samples were collected.9patients with plateau erythropoiesis and 16controls were selected for DNA extraction from feces and analyzed by 16SrRNA sequencing.Results The hemoglobin levels as well as the erythrocyte specific volume were significantly higher in patients with metachromatic erythropoiesis than in the control group.The overall structure of the gut microbiota in patients with erythrocytosis was not significantly altered,and there were no significant differences in diversity.The gut microbiota of patients with erythrocytosis showed a significant increase in the abundance of norank_o_Clostridia_vadinBB60_group,Fusicatenibacter,Anaerostipes,and Eubacterium_hallii_group in Firmicutes;Pseudomonasin Proteobacteria;and Parabacteroidesin Bacteroidetes.These genus alterations were shown to be associated with metabolic disorders by KEGG prediction analysis;Spearman′s analysis showed that multiple genera alterations were positively associated with hemoglobin levels as well as erythrocyte specific volume.Conclusion Patients with plateau erythropoietic disorders have a disturbed gut microbiota,which are associated with metabolic dysbiosis and erythropoiesis.
