磁共振波谱成像检查对前列腺癌的诊断价值探讨

The diagnostic value of magnetic resonance spectroscopy for prostate cancer

目的探究磁共振波谱成像(MRS)检查对前列腺癌的诊断价值。方法选取45例经病理活检证实为前列腺癌患者作为研究组,另选取45例前列腺增生患者作为对照组。所有患者均于入院后进行磁共振成像(MRI)检查、MRS检查,以病理活检作为参照,对比MRI、MRS、MRI+MRS对前列腺癌的诊断准确率、敏感度、特异度、正确指数(约登指数);对比研究组和对照组胆碱(Cho)+肌酸(Cre)/枸橼酸盐(Cit)(CC/C值)。结果研究组CC/C值为(2.15±0.39),高于对照组的(0.65±0.16),差异有统计学意义(P<0.05)。MRI+MRS诊断准确率、敏感度、特异度分别为94.44%、93.33%、95.56%,均高于MRI的58.89%、57.78%、60.00%和MRS的78.89%、77.78%、80.00%,差异有统计学意义(P<0.05);MRS诊断准确率、敏感度、特异度均高于MRI,差异有统计学意义(P<0.05)。MRI+MRS诊断的正确指数大于MRI、MRS单独诊断,MRS诊断的正确指数大于MRI单独诊断。结论MRS能够提升前列腺癌诊断准确率、敏感度、特异度、正确指数,MRI联合MRS能够进一步提升诊断价值,建议推广使用。

Objective To investigate the diagnostic value of magnetic resonance spectroscopy(MRS)for prostate cancer.Methods 45 patients with prostate cancer confirmed by pathological biopsy were selected as the research group,and 45 patients with benign prostatic hyperplasia were selected as the control group.All patients were examined by magnetic resonance imaging(MRI)and MRS after admission.Pathological biopsy was taken as a reference to compare the diagnostic accuracy,sensitivity,specificity,and Yorden index of MRI,MRS,and MRI+MRS for prostate cancer.The CC/C value=choline(Cho)+creatine(Cre)/citrate(Cit)(CC/C value)of the research group and the control group was compared.Results The CC/C value of the research group was(2.15±0.39),which was higher than(0.65±0.16)of the control group,and the difference was statistically significant(P<0.05).The diagnostic accuracy,sensitivity,specificity,and Yorden index of MRI+MRS were 94.44%,93.33%,95.56%,respectively,which were all higher than 58.89%,57.78%,60.00%of MRI,and 78.89%,77.78%,80.00%of MRS,and the difference was statistically significant(P<0.05).The diagnostic accuracy,sensitivity,and specificity of MRS were higher than those of MRI,and the difference was statistically significant(P<0.05).The Yorden index of MRI+MRS was greater than that of MRI and MRS alone,and the Yorden index of MRS was greater than that of MRI alone.Conclusion MRS can improve the diagnostic accuracy,sensitivity,specificity,and Yorden index of prostate cancer.MRI in combination with MRS can further improve the diagnostic value,and it is recommended to be promoted and applied.
