沙库巴曲缬沙坦钠治疗老年慢性心力衰竭合并肾功能不全患者的疗效

Efficacy of sacubitril valsartan sodium in the treatment of elderly patients with chronic heart failure and renal insufficiency

目的分析沙库巴曲缬沙坦钠治疗老年慢性心力衰竭合并肾功能不全患者的疗效。方法30例老年慢性心力衰竭合并肾功能不全患者,随机分为对照组及干预组,每组15例。对照组患者给予常规药物治疗,干预组患者在对照组基础上给予沙库巴曲缬沙坦钠治疗。比较两组患者治疗前后纽约心脏病协会(NYHA)分级、肾功能指标(24 h尿蛋白定量、血肌酐、尿素氮)、左心室射血分数、疗效、不良反应发生情况。结果治疗后,干预组NYHA分级(1.56±0.21)级、24 h尿蛋白定量(81.67±5.21)g/24 h、血肌酐(89.21±11.71)μmol/L、尿素氮(4.22±0.52)mmol/L低于对照组的(2.52±0.56)级、(89.12±7.41)g/24 h、(109.12±23.12)μmol/L、(5.21±0.94)mmol/L,左心室射血分数(56.19±3.42)%高于对照组的(51.24±3.41)%,差异有统计学意义(P<0.05)。干预组患者总有效率100.00%(15/15)高于对照组的66.67%(10/15),差异有统计学意义(P<0.05)。两组均未出现严重不良反应。结论沙库巴曲缬沙坦钠联合常规药物治疗老年慢性心力衰竭合并肾功能不全的临床效果显著,对心功能和肾功能改善明显,值得推广。

Objective To analyze the efficacy of sacubitril valsartan sodium in the treatment of elderly patients with chronic heart failure and renal insufficiency.Methods A total of 30 elderly patients with chronic heart failure and renal insufficiency were randomly divided into the control group and the intervention group,with 15 cases in each group.Patients in the control group were treated with conventional drugs,and patients in the intervention group were treated with sacubitril valsartan sodium on the basis of the control group.Both groups were compared in terms of New York Heart Association(NYHA)classification,renal function indexes(24-h urinary protein quantification,blood creatinine,blood urea nitrogen)and left ventricular ejection fraction before and after treatment,efficacy,and occurrence of adverse reactions.Results After treatment,NYHA classification(1.56±0.21)grades,24-h urinary protein quantitative(81.67±5.21)g/24 h,blood creatinine(89.21±11.71)μmol/L,blood urea nitrogen(4.22±0.52)mmol/L of the intervention group were lower than(2.52±0.56)grades,(89.12±7.41)g/24 h,(109.12±23.12)μmol/L,(5.21±0.94)mmol/L of the control group,and the left ventricular ejection fraction(56.19±3.42)%was higher than(51.24±3.41)%of the control group.All the differences were statistically significant(P<0.05).The total effective rate 100.00%(15/15)in the intervention group was higher than 66.67%(10/15)in the control group,and the difference was statistically significant(P<0.05).No serious adverse reactions were observed in both groups.Conclusion Sacubitril valsartan sodium in combination with conventional drugs has significant clinical effect in the treatment of elderly patients with chronic heart failure and renal insufficiency,which can significantly improve cardiac and renal function of patients.It is worth promoting.
