膝关节前后交叉韧带损伤MRI不同序列诊断分析 被引量:2

摘要 目的:分析膝关节前后交叉韧带损伤分别实施不同序列核磁共振(MRI)检查的诊断效果差异。方法:回顾性分析在本院开展膝关节前后交叉韧带损伤检查的60例病患临床资料,研究时间均在2020年1月至2021年1月之间,所有病患均被确诊为膝关节前后交叉韧带损伤。全部病患均先后实施磁共振三维序列以及二维序列检查,相较于解剖测量值。对比两种检查序列在后交叉韧带(PCL)以及前交叉韧带(ACL)方面测量值;对比诊断情况;对比两种检查序列在膝关节前后交叉韧带损伤分级判断情况;对比两种检查序列在膝关节前后交叉韧带完全撕裂诊断符合率。结果:在PCL以及ACL方面,二维MRI检查角度、厚度、长度均与MRI检查、解剖测量存在显著差异(P<0.05);在PCL以及ACL方面,三维MRI检查角度、厚度、长度均与解剖测量无显著差异(P>0.05)。二维MRI检查诊断准确率是83.33%,低于三维MRI检查的95.00%(P<0.05)。两种检查序列在膝关节前后交叉韧带损伤分级对比无显著差异(P>0.05)。三维MRI检查与二维MRI检查膝关节前后交叉韧带完全撕裂诊断符合率分别是95.55%、80.00%(P<0.05)。结论:膝关节前后交叉韧带损伤病患选择三维MRI检查能够获得较高的诊断价值。 Objective:To analyze the diagnostic effect of different sequences of magnetic resonance imaging(MRI)in anterior and posterior cruciate ligament injury of knee joint.Methods:the clinical data of 60 patients with anterior and posterior cruciate ligament injury in our hospital were retrospectively analyzed.The study time was from January 2020 to January 2021.All patients were diagnosed with anterior and posterior cruciate ligament injury.Three dimensional and two-dimensional sequences of MRI were performed in all patients.The posterior cruciate ligament(PCL)and anterior cruciate ligament(ACL)measured by the two sequences were compared;Diagnosis;The classification of cruciate ligament injury before and after the knee joint and the compliance rate of full tear before and after the knee joint between the two examination sequences.Results:in terms of PCL and ACL,the angle,thickness and length of two-dimensional MRI were significantly different from those of MRI and anatomical measurement(P<0.05);In terms of PCL and ACL,there was no significant difference in angle,thickness and length between 3 D MRI and anatomical measurement(P>0.05).The diagnostic accuracy of two-dimensional MRI was 83.33%,which was lower than 95.00%of three-dimensional MRI(P<0.05).There was no significant difference in the comparison of the anterior and posterior ACL injury classification of the knee joint(P>0.05).The diagnosis of complete anterior and posterior ACL tears was 95.55%and80.00%(P<0.05).Conclusion:three dimensional MRI can obtain high diagnostic value in patients with anterior and posterior cruciate ligament injury.
