洼田饮水试验用于评估全身麻醉患者术后早期进水可行性的研究

Application of Kubota drinking test in the assessment of early water intake in patients under general anesthesia

目的通过VAS口渴评分和洼田饮水试验,从患者舒适性、安全性等方面探讨全身麻醉患者术后早期进水的可行性,为缩短术后禁饮时间提供临床依据。方法选取2019年3月—2019年11月于襄阳市第一人民医院术后入PACU的全身麻醉患者200例,进行VAS口渴评分,于术后0.5、1.0、1.5、2.0 h分别进行洼田饮水试验,并对患者吞咽功能进行评估,记录相关不良反应发生情况。结果患者入PACU后VAS口渴评分(6.3±1.2)分,2 h内4次饮水试验共饮温水约120 ml,出PACU时患者VAS口渴评分为(3.7±0.7)分,差异有统计学意义(P<0.05)。男性VAS口渴评分高于女性(P<0.05);45岁以下的青年患者VAS口渴评分高于45岁以上的中老年患者(P<0.05)。全身麻醉术后2 h患者吞咽功优良率99.5%,未发生严重呕吐误吸事件。结论术后2 h饮水具有一定的可行性。

Objective To discuss the feasibility of early water intake from the aspects of comfort and safe for patients after surgery under general anesthesia through Visual Analogue Scale(VAS)thirsty score and the Kubota drinking water test,so as to pro⁃vide clinical evidence for shorting the time of postoperative drinking deprivation.Methods A total of 200 patients who underwent surgery under general anesthesia in the First People's Hospital of Xiangyang before entry into post anesthesia care unit(PACU)from March 2019 to November 2019 were enrolled.Their VAS thirsty scores were evaluated,and the Kubota drinking water test was per⁃formed 0.5,1.0,1.5 h and 2.0 h after surgery.Their swallowing functions were assessed and adverse reactions were recorded.Re⁃sults Their VAS thirsty score was(6.3±1.2)after entered into PACU,and drank 120 ml of water in four water drinking test within 2 h;while their VAS thirsty score was(3.7±0.7)when discharge from PACU,with statistical difference(P<0.05).Men had stronger feeling of thirsty than women(P<0.05).Patients aged below the age of 45 showed stronger feeling of thirsty than elder people aged above the age of 45(P<0.05).Furthermore,99.5%of the patients passed the drinking water test 2 h after surgery.No serious vomiting was reported.Conclusion Drinking water 2 h after surgery is feasible.
