剖宫产用于二次妊娠合并瘢痕子宫产妇的临床研究

Clinical Study of Cesarean Section for Second Pregnancy with Scar Uterus

目的分析剖宫产在二次妊娠合并瘢痕子宫产妇中的临床应用效果。方法将本院于2019年1月至2020年12月接受治疗的二次妊娠合并瘢痕子宫孕妇80例作为本次观察组,另将同一时期接收的采取剖宫产的非瘢痕子宫妊娠孕妇80例作为本次对照组,分析两组孕妇分别分娩后的手术时间、术中出血量、恶露持续时间、并发症发生率。结果①分析两组孕妇手术时间、术中出血量、恶露持续时间:相较于观察组,对照组孕妇手术时间以及术后恶露持续时间明显更短,术中出血量更少,两组患者对比差异明显(P<0.05)。②分析两组孕妇并发症发生率:与观察组孕妇相比较,对照组孕妇盆腔粘连、子宫切除以及感染发生率明显更低,两组患者对比差异明显(P<0.05)。结论二次妊娠合并瘢痕子宫孕妇在剖宫产手术中出血量更多,手术时间以及术后恶露持续时间更长,术后并发症发生率明显更高,因此,在二次妊娠合并瘢痕子宫产妇的临床治疗中应根据孕妇时间病情对孕妇采取有效的分娩措施,保证母婴安全。

Objective To analyze the clinical effect of cesarean section in the second pregnancy with scar uterus.Methods From January 2019 to December 2020,80 cases of pregnant women with second pregnancy and scarred uterus in our hospital were selected as the observation group,and 80 cases of pregnant women with non scarred uterus who received cesarean section in the same period were selected as the control group.The operation time,intraoperative blood loss,duration of lochia,and complication rate of the two groups of pregnant women after delivery were analyzed.Results①The operation time,intraoperative blood loss and lochia duration of the two groups were analyzed:Compared with the observation group,the operation time and postoperative lochia duration of the control group were significantly shorter,the intraoperative blood loss was less,and the difference between the two groups was significant(P<0.05).②The incidence of complications in the two groups was analyzed:Compared with the observation group,the incidence of pelvic adhesion,hysterectomy and infection in the control group was lower,and the difference between the two groups was significant(P<0.05).Conclusion The second pregnancy with scar uterus pregnant women in cesarean section operation more blood output,operation time and postoperative lochia longer,postoperative complication rate is significantly higher,therefore,in the clinical treatment of second pregnancy with scar uterus pregnant women should take effective delivery measures according to the time and condition of pregnant women,to ensure the safety of mother and baby.
