经鼻罩无创机械通气与经口鼻罩无创机械通气在呼吸衰竭病人中应用效果

Noninvasive Mechanical Ventilation via Nasal Mask versus Transoral Nasal Mask in Patients with Respiratory Failure

目的探讨经鼻罩无创机械通气与经口鼻罩无创机械通气在呼吸衰竭病人中应用效果.方法将2019年12月至2020年3月间60例连州恒生医院收治的呼吸衰竭患者按照随机数字表法分为两组,对照组30例予以经口鼻罩无创机械通气治疗,研究组30例予以经鼻罩无创机械通气治疗.比较两组患者生命体征(血压、呼吸频率、心率、脉搏血氧饱和度)、动脉血气指标[血氧分压(PaO_(2))、二氧化碳分压(PaCO_(2))、pH值、氧合指数]以及患者配合度.结果研究组配合良好比例高于对照组(P<0.05).两组患者通气前血压、呼吸频率、心率、脉搏血氧饱和度比较无显著性差异(P>0.05),通气3h时研究组心率低于对照组,脉搏血氧饱和度高于对照组(P<0.05).两组患者通气前氧合指数、pH值、PaO_(2)、PaCO_(2)比较差异无统计学意义(P>0.05),通气3h时研究组氧合指数、pH值、PaO_(2)高于对照组,PaCO_(2)低于对照组(P<0.05).结论经鼻罩无创机械通气治疗呼吸衰竭患者相较于经口鼻罩无创机械通气治疗而言患者配合度更高,更有利改善患者生命体征及动脉血气指标.

Objective To investigate the effect of noninvasive mechanical ventilation via nasal mask versus transoral nasal mask in patients with respiratory failure.Methods Sixty patients with respiratory failure admitted to Lianzhou Hengsheng Hospital between December 2019 and March 2020 were divided into two groups according to the random number table method,30 in the control group were treated with non-invasive mechanical ventilation by nasal mask,and 30 in the study group were treated with non-invasive mechanical ventilation by nasal mask.Vital signs(blood pressure,respiratory rate,heart rate,pulse oximetry),arterial blood gas indices[partial pressure of oxygen(PaO_(2)),partial pressure of carbon dioxide(PaCO_(2)),pH,oxygenation index],and patient cooperation were compared between the two groups.Results The proportion of patients with a good fit was higher in the study group than in the control group(P<0.05).Blood pressure,respiratory rate,heart rate,and pulse oximetry were not significantly different between the two groups(P>0.05),and heart rate was lower and pulse oximetry higher in the study group than in the control group at 3 h of ventilation(P<0.05).The oxygenation index,pH,PaO_(2),and PaCO_(2) of the study group before ventilation were not significantly different between the two groups(P>0.05),but at 3h of ventilation the oxygenation index,pH,and PaO_(2) of the study group were higher than those of the control group,and the PaCO_(2) was lower than that of the control group(P<0.05).Conclusion Patients with respiratory failure treated with nasal mask noninvasive mechanical ventilation have a higher degree of cooperation and a more favorable improvement of patients’vital signs and arterial blood gas indicators than patients treated with nasal mask noninvasive mechanical ventilation.
