银荷漱口方口腔护理对预防老年肺部感染患者口腔真菌感染的影响

Effect of Oral Care with Yinhe Gargling Formula on Prevention of Oral Fungal Infection in Elderly Patients with Pulmonary Infection

目的探讨银荷漱口方口腔护理对预防老年肺部感染患者口腔真菌感染的影响.方法选取江门市新会区中医院2020年1月至2021年2月收治的50例老年肺部感染患者,按照随机数字表法分为对照组(n=25,使用生理盐水作为口腔护理液)每日以棉球蘸取少量生理盐水擦拭患者口腔,按照基础护理程序要求,2次/d,叮嘱患者每餐进食后以温开水进行漱口.观察组(n=25,采用自拟中药漱口方[银河漱口方]漱口)漱口方组成如下:银花30g、板蓝根30g、薄荷10g、甘草15g、生地30g、杭菊20g,由本院外送代煎至100mL,每日一剂.口腔护理方法与对照组方案相同.比较两组患者住院期间口腔真菌感染发生率、口腔评分及口腔溃疡发生情况.结果干预前,两组患者的真菌感染率及口腔评分差异无统计学意义(P>0.05),干预3天,7天后,两组患者真菌感染率及口腔评分均有所下降,其中观察组明显低于对照组(P<0.05);观察组干预3天后口腔溃疡发生率为20.00%低于对照组32.00%,干预7天口腔溃疡发生率为12.00%低于对照组32.00%(P<0.05).结论银荷漱口方口腔护理可维护老年肺部感染患者口腔健康,降低口腔真菌感染发生率、降低口腔评分,减少口腔溃疡的发生.

Objective To investigate the effect of oral care with Yinhe gargling formula on the prevention of oral fungal infection in elderly patients with pulmonary infection.Methods Fifty elderly patients with pulmonary infection who were admitted to Jiangmen Xinhui District Hospital of Traditional Chinese Medicine,from January 2020 to February 2021 were divided into control group(n=25,using normal saline as oral care solution)according to the random number table method,taking a small amount of normal saline daily in a cotton ball dip to wipe the patient’s mouth,and according to the requirements of the basic care process,the patients were instructed to swish with warm water after each meal.The observation group(n=25,gargling formula using self-designed traditional Chinese medicine gargling formula[Yinhe gargling formula])consisted of the following:Yinhua 30 g,bluish root 30 g,peppermint 10 g,Gancao 15 g,Shengdi 30 g,hangju 20 g,and decocted decoction was delivered out of our hospital to 100 mL once daily.The methods of oral care were identical to the control group protocol.The incidence of oral fungal infection,oral score and occurrence of oral ulcers during hospitalization were compared between the two groups.Results Before the intervention,there was no significant difference in the fungal infection rate and oral score between the two groups(P>0.05),after 3 days and 7 days of the intervention,the fungal infection rate and oral score decreased in both groups,in which the observation group was significantly lower than the control group(P<0.05),the incidence of oral ulcer in the observation group after 3 days of the intervention was 20.00%lower than that in the control group(32.00%)and the incidence of oral ulcer in the intervention group after 7 days was 12.00%lower than that in the control group32.00%in the control group(P<0.05).Conclusion The oral care of Yinhe gargling formula can maintain the oral health of elderly patients with pulmonary infection,reduce the incidence of oral fungal infection,reduce the oral score,and reduce the incidence of oral ulcers.
