GeneXpert MTB/RIF检测结核性脑膜炎患者 脑脊液标本的临床应用价值 被引量:1

Clinical application value of GeneXpert MTB/RIF in detecting cerebrospinal fluid samples from patients with tuberculous meningitis

摘要 目的探讨GeneXpert MTB/RIF(以下简称“GeneXpert”)检测结核性脑膜炎患者的脑脊液应用价值。方法选取2014年12月至2019年12月在沈阳市第十人民医院住院治疗的初诊为结核性脑膜炎患者120例作为研究对象,收集患者的脑脊液标本,进行脑脊液抗酸杆菌涂片检测、脑脊液结核分枝杆菌960培养及脑脊液GeneXpert检测,分析GeneXpert的检测效能。结果脑脊液抗酸杆菌检测阳性9例,阴性111例;脑脊液结核分枝杆菌960培养阳性40例,阴性80例;脑脊液GeneXpert检测阳性39例,阴性81例。以脑脊液涂片为标准,GeneXpert检测的灵敏度及特异度分别为100.00%和72.97%;以脑脊液960培养为标准,GeneXpert检测的灵敏度及特异度分别为97.50%和100.00%;以临床诊断为标准,GeneXpert检测的灵敏度及特异度分别为95.12%和100.00%。结论脑脊液GeneXpert检测灵敏度和特异度较高、检测时间短,有利于临床早期诊断结核性脑膜炎。 Objective To investigate the value of GeneXpert MTB/RIF(hereinafter referred to as"GeneXpert")in detecting cerebrospinal fluid(CSF)in patients with tuberculous meningitis.Methods A total of 120 newly diagnosed tuberculous meningitis patients hospitalized in Shenyang Tenth People's Hospital from December 2014 to December 2019 were selected as the study subjects.Cerebrospinal fluid samples were collected for cerebrospinal fluid acid-fast bacilli smear detection,cerebrospinal fluid Mycobacterium tuberculosis 960 culture and cerebrospinal fluid GeneXpert detection.The detection efficacy of GeneXpert was analyzed.Results 9 cases were positive,and 111 cases were negative for acid-fast bacilli.Forty cases were positive,and 80 cases were negative for Mycobacterium tuberculosis 960 culture.Thirty-nine cases were positive,and 81 cases were negative for the GeneXpert test.Using cerebrospinal fluid smear as the standard,the sensitivity and specificity of GeneXpert detection were 100.00%and 72.97%,respectively.Using cerebrospinal fluid 960 culture as the standard,the sensitivity and specificity of GeneXpert detection were 97.50%and 100.00%,respectively.And using clinical diagnosis as the standard,the sensitivity and specificity of GeneXpert detection were 95.12%and 100.00%,respectively.Conclusion The GeneXpert detection in cerebrospinal fluid has high sensitivity and specificity,and a short detection time,which is helpful for early clinical diagnosis of tuberculous meningitis.
