子午流注法针刺联合四君子汤加减治疗脾虚湿盛型溃疡性结肠炎临床观察

Clinical Observation on Treating Ulcerative Colitis of Spleen Deficiency and Excessive Dampness Type by Midnight-Noon Ebb-Low Acupuncture Combined with Sijunzi Decoction(四君子汤)

目的:观察基于子午流注理论的针刺联合四君子汤加减治疗脾虚湿盛型溃疡性结肠炎的临床疗效。方法:以120例脾虚湿盛型溃疡性结肠炎患者为研究对象,随机分成针刺组、中药组、联合组三组,每组40例,分别予以常规针刺、四君子汤加减、子午流注法针刺联合四君子汤治疗。比较三组的临床疗效、临床症状与体征评分、结肠镜下黏膜变化(采用Baron评分)与组织病理变化情况。结果:联合组总有效率95.0%,疗效优于针刺组与中药组(P<0.05);治疗后三组患者各项症状体征评分均有明显降低(P<0.05),且联合组较其他两组下降更为显著(P<0.05);治疗后三组患者Baron评分、结肠组织病理变化评分均有明显降低(P<0.05),且联合组较其他两组下降更为显著(P<0.05)。结论:基于子午流注理论的针刺联合四君子汤加减治疗脾虚湿盛型溃疡性结肠炎可抑制机体产生的过度免疫反应,增强肠黏膜保护作用,有效改善肠道功能,缓解临床症状。

Objective:To observe the clinical effect of midnight-noon ebb-low acupuncture combined with Sijunzi Decoction(四君子汤)on ulcerative colitis of spleen deficiency and excessive dampness type.Methods:One hundred and twenty patients with ulcerative colitis of spleen deficiency and excessive dampness type were randomly divided into three groups,acupuncture group,Chinese medicinals group and combination group,40 patients in each group,treated with routine acupuncture,modified Sijunzi Decoction,midnight-noon ebb-low acupuncture combined with Sijunzi Decoction,respectively.The clinical efficacy,clinical symptoms and body sign scores,mucosal changes under colonoscopy(Baron scores)and pathological changes were compared.Results:The total effective rate of combination group was 95.00%,with better efficacy than that of acupuncture group and Chinese medicinals group(P<0.05).After treatment,the clinical symptoms and body sign scores were decreased in three groups(P<0.05),and ompared with the other two groups,these scores in combination group decreased more significantly(P<0.05).After treatment,Baron scores and pathological changes scores were decreased obviously among three groups(P<0.05),the decrease was more significant in the combined group compared with the other two groups.Conclusion:Midnight-noon ebb-low acupuncture combined with modified Sijunzi Decoction for ulcerative colitis of spleen deficiency and excessive dampness type could significantly inhibit the excessive immune response of the body,enhance the protective effect of intestinal mucosa,and effectively improve the intestinal function and clinical symptoms.
