扬州地区838例泌尿系结石成分及相关因素分析 被引量:8

Analysis of urinary calculi composition and related factors in Yangzhou city:aprospective study of 838cases

摘要 目的:分析扬州地区泌尿系结石成分构成及分布特征,探讨尿路结石的高发因素及临床防治措施。方法:采用红外光谱自动分析仪,对本院2018年9月—2020年9月收治的838例患者的泌尿系结石标本进行成分分析,并结合临床资料研究其分布特征。结果:本组838例尿石症患者中男女比例为3.23:1,年龄16~95岁,平均(53.5±14.3)岁,其中中年(41~65岁)患者最多,共499例(59.6%);上、下尿路结石比为2.92:1,男性上尿路结石占比67.5%,女性上尿路结石占比97.0%。单纯性结石469例,以草酸钙为主,在所有结石成分中占34.7%,其次为尿酸结石(15.8%);混合性结石369例,以草酸钙+磷酸钙结石为主,占全部结石的33.5%。可获取CT影像资料患者604例,尿酸结石平均CT值为(492.16±144.46)HU,草酸钙、草酸钙+磷酸钙、磷酸钙结石平均CT值均在900~1000 HU;青年患者多见草酸钙+磷酸钙结石(51.3%),中年患者以草酸钙结石(37.5%)为主,老年患者多发尿酸(36.1%)结石。结论:扬州地区泌尿系结石成分分布有其自身特点,根据本研究对扬州地区结石成分分布及相关因素的分析,可为防治扬州地区尿石症及预测患者结石成分提供参考依据。 Objective:To analyze the composition of urinary calculi and related factors in 838cases of Yang-zhou,Jiangsu Province,and explore the high risk factors of urolithiasis formation and the clinical prevention and treatment measures for urinary calculi.Methods:The specimens of urinary calculi in 838patients diagnosed with urolithiasis in our hospital from September 2018to September 2020were analyzed by the infrared spectrum stone composition analyzer,and the distribution characteristics were analyzed combined with the clinical data of all pa-tients.Results:Among 838patients of urinary calculi,the ratio of male to female was 3.23:1,and aged between 16and 95years old,mean of(53.5±14.3)years.All patients with high prevalence were middle-aged(41-65 years old),with a total of 499cases(59.6%).The ratio of upper and lower urinary tract stones was 2.92:1,male upper urinary tract stones accounted for 67.5%,and in the female patients,the proportion of upper urinary tract stones was 97.0%.The main component of pure stones(469cases)was calcium oxalate,accounting for 34.7%of all stones,followed by uric acid stones(15.8%);the component of mixed stones was dominated by cal-cium oxalate+calcium phosphate,accounting for 33.5%of all stones.CT imaging data of 604patients were a-vailable.The average CT value of uric acid stones was(492.16±144.46)HU,and the average CT values of calci-um oxalate,calcium oxalate+calcium phosphate,and calcium phosphate stones were all between 900and 1000 HU.Calcium oxalate+calcium phosphate stones were more common in young patients(51.3%),calcium ox-alate stones(37.5%)were predominant in middle-aged patients,and uric acid stones(36.1%)were common in elderly patients.Conclusion:The distribution of urinary stones in Yangzhou area has its own characteristics.Ac-cording to the analysis of the distribution of urinary calculi components and related factors,it can provide reference for preventing and treating urolithiasis in Yangzhou area and predicting the composition of urinary calculi in pa-tients.
