摘要
目的:探讨多次输血者,TRALI疑似患者,正常献血者等三类人群机体内与免疫性TRALI相关的HLA抗体和HNA抗体种类及其水平的概况。方法:利用HLA和HNA检测试剂盒对100例三组人群进行HLA抗体和HNA抗体检测以及特异性的鉴定。结果:多次输血者中检出HLAⅠ类抗体24例(72.73%),检出HLAⅡ类抗体21例(63.64%);TRALI疑似患者检出HLAⅠ类抗体7例(15.56%),检出HLAⅡ类抗体12例(26.67%);正常献血者检出HLAⅠ类抗体4例(18.18%),检出HLAⅡ类抗体2例(9.09%)。三类人群共检出有HNA抗体的标本20例,其中多次输血者11例(33.33%),TRALI疑似患者检出HNA抗体7例(15.55%),正常献血者检出HNA抗体2例(9.10%)。结论:在多次输血者中HLA抗体和HNA抗体发生率最高,其次是TRALI疑似患者,正常献血者最低。因而在临床实践中,需重视对多次输血者和TRALI(疑似)患者HLA抗体和HNA抗体的筛查工作,从而保障用血安全。
Objective: To analyze the types and levels of HLA antibodies and HNA antibodies associated with immune TRALI among three groups of patients with multiple transfusions, TRALI suspected patients and normal blood donors. Methods: A total of 100 samples were randomly collected from the three different groups and tested for HLA antibodies and HNA antibodies, and then identified specificity by screening kit. Results: A total of 24 cases(72.73%) HLAⅠ antibodies and 21 cases(63.64%) HLA Ⅱ antibodies were detected in patients with multiple transfusions, 7 cases(15.56%) HLAⅠ antibodies and 12 cases(26.67%) HLAⅡantibodies were found in TRALI suspected patients, 4 cases(18.18%) HLAⅠ antibodies and 2 cases(9.09%) HLAⅡantibodies were screened in normal blood donors. There were 20 samples with HNA antibodies in three groups, including 11 cases multiple transfusions(33.33%), 7 cases HNA antibodies TRALI suspected patients(15.55%) and 2 HNA antibodies(9.10%) in normal blood donors. Conclusion: The positive rate of HLA antibodies and HNA antibodies was the highest among patients with multiple transfusions, followed by TRALI suspected patients and the lowest among normal blood donors. Therefore, in clinical practice, we should pay attention to the screening of HLA antibodies and HNA antibodies in the patients with multiple transfusions and TRALI(suspected) patients, so as to ensure the safety of blood use.
