舒肝解郁胶囊对喹硫平血药浓度影响研究

Effect of Shugan Jieyu Capsules combined with quetiapine on the serum concentration of quetiapine

目的探究舒肝解郁胶囊对精神疾病患者喹硫平血药浓度的影响。方法回顾性收集服用喹硫平治疗的住院患者的病历资料,记录患者的一般信息、给药方案、肝功能指标以及喹硫平稳态谷浓度等,根据是否联用舒肝解郁胶囊分为试验组(喹硫平联用舒肝解郁胶囊)和对照组(喹硫平)。结果共纳入116例样本信息,其中试验组和对照组的喹硫平血药浓度值分别为(90.76±14.51)和(169.01±15.27)ng·mL^(-1),剂量校正浓度分别为(0.24±0.04)和(0.47±0.05)ng·mL^(-1)·mg^(-1)·d;试验组和对照组超出治疗窗的比例为70.97%(22例次/31例次)和43.53%(37例次/85例次),差异均有统计学意义(均P<0.01)。结论舒肝解郁胶囊与喹硫平合用能显著降低喹硫平的血药浓度,建议两药合用时适当增加喹硫平的剂量,加强喹硫平血药浓度监测,促进药物临床合理应用。

Objective To explore the effect of Shugan Jieyu Capsule on the serum concentration of quetiapine in patients with mental illness.Methods A retrospective study of inpatients who were treated with quetiapine was collected.The general information of the patients,dosing regimens,liver function indicators,and steady-state trough concentrations of quetiapine were recorded.It was divided into treatment group(quetiapine combined with Shugan Jieyu Capsule)and control group(quetiapine),according to whether combined with Shugan Jieyu Capsule or not.Results A total of 116 samples were included.The serum concentration of quetiapine in treatment group and control group were(90.76±14.51)and(169.01±15.27)ng·mL^(-1),respectively,the dose-related concentration were(0.24±0.04)and(0.47±0.05)ng·mL^(-1)·mg^(-1)·d,respectively.The ratio of treatment group and control group beyond the treatment window were 70.97%(22 cases/31 cases)and 43.53%(37 cases/85 cases),respectively.The difference was statistically significant between the two groups(all P<0.01).Conclusion The combination of Shugan Jieyu Capsules with quetiapine can significantly reduce the serum concentration of quetiapine.Clinicians may need to consider increasing the recommended daily dose when the two drugs were combined,and the therapeutic drug monitoring of quetiapine should be carried out to promote the rational clinical app lic ation of the drugs.
