摘要 目的探讨中心动脉压与高血压患者早期肾损伤的相关性。方法选取2018年1月至2019年6月在嘉兴市第一医院心内科住院拟行冠状动脉造影的高血压患者206例,根据肌酐清除率(CCr)分为肾损伤组(60 ml/min<CCr<90 ml/min)110例和对照组(CCr≥90 ml/min)96例,测定两组患者的中心动脉收缩压(CSBP)、中心动脉舒张压(CDBP)、尿微量白蛋白(UMA)。采用Pearson相关及多元线性逐步回归法分析CSBP与UMA的关系。结果肾损伤组CSBP、UMA均高于对照组(均P<0.05),CSBP与UMA呈正相关(r=0.29,P<0.01),多元线性逐步回归分析显示CSBP是UMA的独立危险因素(β=0.37,t=7.04,P<0.01)。结论高血压患者CSBP升高是引起UMA升高的独立危险因素,对高血压患者早期肾脏损伤的判断有一定的价值。 Objective To investigate the relationship between central arterial pressure and early renal injury in patients with hypertension.Methods A total of 206 patients with hypertension who underwent coronary angiography in the First Hospital of Jiaxing were enrolled and patients were divided into kidney injury groups(60 ml/min<CCr<90 ml/min)and control group(CCr≥90 ml/min).The central arterial systolic blood pressure(CSBP),central arterial diastolic blood pressure(CDBP),and urine microalbumin(UMA)were determined.The relationship between CSBP and UMA was analyzed by multivariate regression analysis.Results Both CSBP and UMA were higher in renal injury group than those in control group(all P<0.05),CSBP was positively correlated with UMA(r=0.29,P<0.01).The multivariate stepwise regression analysis showed that CSBP was an independent risk factor for UMA(β=0.37,t=7.04,P<0.01).Conclusion Elevated CSBP is an independent risk factor for elevated UMA in patients with hypertension,indicating that CSBP may be used to predict early renal injury in hypertensive patients.
