磁共振TSE-DWI序列在鼻咽癌检查中的应用价值 被引量:4

A preliminary study on the application value of TSE-DWI sequence in imaging nasopharyngeal carcinoma

摘要 目的探讨在鼻咽癌患者中,比较快速自旋回波弥散成像(TSE-DWI)、单次激发平面回波弥散成像(SS-EPI-DWI)检查序列的图像质量、ADC值,3.0T磁共振TSE-DWI序列在鼻咽部检查中的应用价值。方法利用3.0T MR扫描仪对50例鼻咽癌患者进行TSE-DWI序列及SS-EPI-DWI序列扫描,比较其成像时间、图像质量主观评分、信噪比(SNR)、对比噪声比(CNR)、瘤灶对比度及表观扩散系数(ADC值)等,并进行统计学分析。结果TSE-DWI图像的磁化率伪影评分、几何变形程度评分、SNR及CNR均优于SS-EPI-DWI(P值均<0.001);TSE-DWI图像中瘤灶的对比度及ADC值与SS-EPI-DWI没有明显差异(P值≥0.001);TSE-DWI与SS-EPI-DWI序列扫描时间分别为2min32s、1min27s,前者扫描时间更长。结论在3.0T磁共振鼻咽部检查中,采用TSE-DWI序列能明显改善DWI图像质量,显著避免图像变形,获得准确的ADC值,但其扫描时间更长。 Objective To compare the image quality and apparent diffusion coefficients(ADCs)values of the modified TSE-DWI sequence with the classic spectral diffusion sequence(SS-EPI-DWI)in nasopharyngeal carcinoma patients.Methods Fifty nasopharyngeal carcinoma patients(29~61 years old;39 males and 11 females)were scheduled,and prospectively underwent a nasopharyngeal 3T MRI scanning with TSE-DWI and SS-EPI-DWI sequence.The ADCs,the signal-to-noise ratios,the contrast-to-noise ratios,and the image quality subjective scores of the two groups were analyzed.The scan time of each sequences was recorded and compared.Results The scores in the magnetic sensitive artifacts and image distortion of TSE-DWI were significantly higher than those of SS-EPI-DWI(all P<0.001).TSE-DWI showed a significantly higher SNR and CNR than SS-EPI-DWI(all P<0.001).There was no significant difference in the lesions contrast and ADC values between the two groups(P>0.001).The scan time of TSE-DWI(2 minutes 32 seconds)was longer than SS-EPI-DWI(1 minute 27 seconds).Conclusion TSE-DWI sequence can improve the image quality for nasopharyngeal carcinoma patients at 3 T.With the use of TSE-DWI sequence,we can avoid the image distortion,especially at nasopharynx.However,TSE-DWI sequence will take longer scan time than SS-EPI-DWI.
