耳内镜下岛状软骨膜复合体内外植法修补鼓膜大穿孔的疗效 被引量:3

Effect Analysis of Island Shaped Cartilage-perichondrium Complex in Repairing Big Tympanic Membrane Perforation by Inner-outer Method under Ear Endoscope

摘要 目的探讨耳内镜下利用岛状软骨膜复合体内外植法修补鼓膜大穿孔的疗效。方法回顾性分析87例(87耳)接受耳内镜手术患者的临床资料,比较分别采用岛状软骨膜复合体(实验组)(n=46)和软骨膜修补鼓膜(对照组)(n=41)治疗的患者术后有效率、并发症及听力恢复情况。结果实验组患者有效率(100.00%)与对照组(97.56%)差异无统计学意义(P>0.05),发生并发症分别为1例、4例。实验组患者术后3个月气导听力较术前提高、气骨导差较术前缩小(P均<0.05);对照组术后3个月气导听力较术前提高、气骨导差较术前缩小(P均<0.05)。结论耳内镜下岛状软骨膜复合体内外植法修补鼓膜大穿孔可获得较好的疗效,术后听力恢复好,并发症发生少。 Objective To explore the therapeutic effect of repairing big tympanic membrane perforation by an island-shaped like complex of cartilage and perichondrium,in an inner-outer method,under ear endoscope.Methods 87 cases(ears)who received ear endoscopy surgeries were enrolled in our study and divided into test group(n=46)and control group(n=41),and they received island shaped like cartilage-perichondrium complex and single perichondrium implantation respectively and compare the efficiency,complications and hearing outcomes between the groups for the first month and the third month after surgery.Results The test group got an effective rate of 100.00%while the control group was 97.56%.The difference was not statistically significant(P>0.05),and one case and four cases had complications in each group.In test group,air conduction hearing and air bone conduction hearing gap were improved three months after surgery than before surgery,the differences were statistically significant(P all<0.05).In control group,air conduction hearing and air bone conduction hearing gap were improved three months after surgery than before surgery,the differences were statistically significant(P all<0.05).Conclusion The repair of large tympanic membrane perforation with island perichondrium combined with in vivo implantation under endoscope can obtain good curative effect,good postoperative hearing recovery and less complications.
