门诊药房调配内差影响因素分析及改进措施

Analysis of influencing factors and improvement measures of internal difference in outpatient pharmacy allocation

目的:探讨门诊药房调配内差的影响因素及改进措施。方法:抽取2020年12月-2021年5月医院处方5000份,对发生差错情况进行统计,整理差错原因,并分类总结。结果:5000份处方中,出现差错65份,差错发生率为1.3%。医生因素21例,占差错总数的32.3%;其中剂量错误10份,药名错误9份,患者混淆2份。药师因素44份,占差错总数的67.7%;其中药品上错药架1份,占差错总数的1.5%;退回药品归位错误1份,占差错总数的1.5%;药品规格相同厂家不同5份,占差错总数的7.75%;发错患者4份,占差错总数的6.2%;用法用量错误4份,占差错总数的6.2%;包装相似4份,占差错总数的6.2%;药品名称相似6份,占差错总数的9.2%;药品规格差错6份,占差错总数的9.2%;药品剂型差错6份,占差错总数的9.2%;药品品种差错7份,占差错总数的10.8%。结论:临床医生和药剂师是门诊药房调配内差的主要因素,所以加强责任心,提高业务水平,完善制度,加大执行力度是降低差错发生的重要措施。

Objective:To explore the influencing factors and improvement measures of internal difference in outpatient pharmacy allocation.Methods:5000 prescriptions were selected from our hospital from December 2020 to May 2021,we made statistics on the errors,sorted out the causes of the errors,and classified and summarized them.Results:Among 5000 prescriptions,65 had errors,and the error rate was 1.3%.There were 21 cases of doctor factors,accounting for 32.3%of the total errors.There were 10 wrong doses,9 wrong drug names and 2 confused patients.There were 44 pharmacist factors,accounting for 67.7%of the total errors.Among them,there was one wrong medicine rack,accounting for 1.5%of the total errors,one returned drug had wrong location,5 copies of drugs with the same specifications and different manufacturers,accounting for 7.75%of the total errors,there were 4 wrong patients,accounting for 6.2%of the total errors,there were 4 errors in usage and dosage,accounting for 6.2%of the total errors,there were 4 similar packages,accounting for 6.2%of the total errors,there were 6 copies with similar drug names,accounting for 9.2%of the total errors,there were 6 errors in drug specifications,accounting for 9.2%of the total errors,there were 6 errors in drug dosage forms,accounting for 9.2%of the total errors,there were 7 errors in drug varieties,accounting for 10.8%of the total errors.Conclusion:Clinicians and pharmacists are the main factors of poor allocation in outpatient pharmacies,strengthening the sense of responsibility,improving the business level,perfecting the system and strengthening the implementation are important measures to reduce the occurrence of errors.
