记忆门诊就诊者神经精神症状观察及认知水平分析

Observation of neuropsychiatric symptoms and analysis of cognitive level in memory outpatients

目的:观察复旦大学附属华东医院记忆门诊患者神经精神症状(NPS)的患病情况,并探讨NPS与认知水平的关系以及对疾病诊治的意义。方法:对2018年9月-2019年12月在记忆门诊就诊的76例患者根据有无精神症状进行分析,并对认知水平进行比较。结果:本研究中轻度认知障碍(MCI)患者出现NPS比例为42.6%,而在痴呆患者中为78.3%。伴NPS患者与无NPS患者首诊年龄、性别、MMSE比较,差异无统计学意义(P>0.05),但CDR分值显著升高。结论:记忆门诊中近半数患者首诊时即有NPS,痴呆患者中出现NPS的比例更高;在相同认知水平下,伴NPS患者功能受损更多;轻度行为损害(MBI)这一概念有助于从神经精神角度及早发现、诊治痴呆。

Objective:To observe the prevalence of neuropsychiatric symptoms(NPS)in memory outpatients of East China Hospital Affiliated to Fudan University,and to explore the relationship between NPS and cognitive level and its significance in disease diagnosis and treatment.Methods:From September 2018 to December 2019,we statistically analyzed 76 patients enrolled from the memory outpatient archives,they were grouped according to whether they had mental symptoms or not,and their cognitive levels were compared.Results:In this study,the proportion of NPS in patients with MCI was 42.6%and 78.3%in patients with dementia.There was no significant difference in age at the first visit and gender composition between patients with NPS and patients without NPS,there was no significant difference in MMSE between patients with NPS and patients without NPS,but the CDR score increased significantly.Conclusion:Nearly half of the patients in the memory clinic has NPS at the first visit,the proportion of NPS is higher in patients with dementia.At the same cognitive level,patients with NPS have more functional impairment.The concept of mild behavioral impairment(MBI)is helpful to the early detection,diagnosis and treatment of dementia from the perspective of neuropsychology.
