益生菌制剂联合母亲声音刺激疗法、间歇持续喂养在极低出生体质量早产儿中的作用探讨 被引量:6

Study on the effect of probiotic preparations combined with maternal voice stimulation therapy and intermittent continuous feeding on premature infants with very low birth mass

摘要 目的分析益生菌制剂联合母亲声音刺激疗法、间歇持续喂养在极低出生体质量早产儿中的作用。方法选取2018年12月—2019年12月在徐州市妇幼保健院出生的极低出生体质量早产儿84例,按照出生后进入监护室顺序进行编号,根据随机数字表法分为观察组和对照组,每组42例(最终共脱失2例)。对照组给予母亲声音刺激疗法+间歇持续喂养治疗,观察组给予益生菌制剂+母亲声音刺激疗法+间歇持续喂养治疗。对比分析两组患儿临床治疗的情况,治疗前后营养学指标的变化,生长发育情况以及患儿喂养不耐受发生率。结果观察组患儿的初次排便时间、胎粪排尽时间、达到完全肠内营养时间以及住院时间均短于对照组(P<0.05);治疗第14天,两组患儿血红蛋白、前白蛋白水平较治疗前均升高(P<0.05),且观察组患儿血红蛋白、前白蛋白水平明显高于对照组(P<0.05);治疗第14天,两组患儿头围、身长及体质量较治疗前均有所增加(P<0.05),且观察组患儿头围、身长及体质量明显大于对照组(P<0.05);观察组患儿的喂养不耐受发生率与对照组比较无统计学差异(P>0.05)。结论益生菌制剂联合母亲声音刺激疗法、间歇持续喂养能够改善极低出生体质量早产儿的胃肠动力情况,加快肠道的营养吸收,缩短住院时间,改善患儿的营养状况,促进患儿生长发育,不增加喂养不耐受发生率。 Objective Analyze the effects of probiotic preparations combined with maternal voice stimulation therapy and intermittent continuous feeding in preterm infants with very low birth weight.Methods A total of 84 preterm infants with very low birth weight who were hospitalized in our hospital from December 2018 to December 2019 were selected,and number them according to the order in which they entered the intensive care unit after birth.According to the random number table method,they were divided into observation group and control group,with 42 cases in each group(a total loss of 2 cases).The control group was given maternal voice stimulation therapy and intermittent continuous feeding,the observation group was given probiotic preparation and maternal voice stimulation therapy and intermittent continuous feeding.The clinical treatment status,the changes in nutritional indicators before and after treatment,growth and development and the incidence of feeding intolerance of children of the two groups were compared.Results The first defecation time,the time to exhaust meconium,the time to complete enteral nutrition,and the length of hospital stay in the observation group were shorter than those in the control group(P<0.05).On the 14th day of treatment,the levels of hemoglobin and prealbumin of the two groups were higher than before treatment(P<0.05),and the levels of hemoglobin and prealbumin of the observation group were substantially higher than those of the control group(P<0.05).On the 14th day of treatment,the head circumference,body length and body weight of the children in the two groups increased compared with those before treatment(P<0.05),and the head circumference,body length and body weight of the children of the observation group were significantly larger than those of the control group(P<0.05).There was no significant difference in the incidence of feeding intolerance between the observation group and the control group(P>0.05).Conclusion Probiotic preparations combined with maternal voice stimulation therapy and intermittent continuous feeding can improve the gastrointestinal motility of preterm infants with very low birth weight,accelerate the absorption of intestinal nutrition,shorten the length of hospitalization,improve the nutritional status of children,promote growth and development,and do not increase incidence of feeding intolerance.
