基于倾向值匹配应激性高血糖与剖宫产术后产褥感染相关性研究

Correlation between Stress Hyperglycemia and Puerperal Infection after Cesarean Section Based on Propensity Value Matching

目的分析应激性高血糖与剖宫产术后产褥感染相关性。方法回顾调查2017年1月-2020年12月行剖宫产术的产妇资料,按1∶1对术后伴应激性高血糖与不伴有应激性高血糖产妇进行匹配,匹配方法采用倾向性评分匹配(PSM)法,比较伴有应激性高血糖与不伴应激性高血糖产妇行剖宫产术后产褥感染率、住院时间与产褥感染治愈时间。结果在5498例行剖宫产术的产妇中出现应激性高血糖472例,应激性高血糖发生率为8.58%。PSM配比并均衡检验后,伴有应激性高血糖产妇产褥感染发生率、住院时间与产褥感染治愈时间均显著高于不伴应激性高血糖产妇(P<0.05);多因素Logistic模型回归分析显示应激性高血糖与产褥感染呈正相关(OR=2.159,95%CI:1.096-4.256)。结论应激性高血糖与剖宫产术后产褥感染呈正相关,应激性高血糖筛查并进行控制对降低术后产褥感染率具有重要意义。

Objective To analyze the correlation between stress hyperglycemia and puerperal infection after cesarean section.Methods The data of cesarean section from January 2017 to December 2020 were retrospectively investigated.The postpartum infection rate,hospital stay and cure time of postpartum infection were compared between the two groups according to the ratio of 1∶1.Results Among the 5498 parturients undergoing cesarean section,472 cases of stress hyperglycemia occurred,and the incidence of stress hyperglycemia was 8.58%.After PSM matching and balanced test,the incidence of puerperal infection,hospital stay and cure time of puerperal infection in women with stress hyperglycemia were significantly higher than those in women without stress hyperglycemia(P<0.05).Multivariate logistic regression analysis showed that stress hyperglycemia was positively correlated with puerperal infection(OR=2.159,95%CI:1.096-4.256).Conclusion Stress hyperglycemia is positively correlated with puerperal infection after cesarean section.Screening and controlling stress hyperglycemia is of great significance to reduce the rate of puerperal infection after cesarean section.
