二甲双胍联合左炔诺孕酮宫内缓释系统治疗子宫内膜不典型增生及早期子宫内膜癌合并代谢综合征的效果

Effect of Metformin Combined with Levonorgestrel Intrauterine Sustained Release System on Endometrial Atypical Hyperplasia and Early Endometrial Cancer Complicated with Metabolic Syndrome

目的分析二甲双胍联合左炔诺孕酮宫内缓释系统(LNG-IUS)治疗子宫内膜不典型增生(AEH)及早期子宫内膜癌(EC)合并代谢综合征(肥胖、胰岛素抵抗和糖尿病)的效果。方法选取2014年6月至2019年6月郑州大学第五附属医院收治的42例AEH及早期EC合并代谢综合征患者作为研究对象,按照随机数表法分为对照组(20例)和观察组(22例)。对照组接受二甲双胍联合孕激素治疗,观察组接受二甲双胍联合LNG-IUS治疗。比较两组治疗效果和不良反应发生情况。结果两组患者治疗总有效率比较,差异无统计学意义(P>0.05);观察组不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论二甲双胍联合LNG-IUS与二甲双胍联合孕激素治疗AEH及早期EC合并代谢综合征效果相当,但二甲双胍联合LNG-IUS副作用更小,患者不良反应发生率低。

Objective To analyze the efficacy of metformin combined with levonorgestrel intrauterine system(LNG-IUS)in the treatment of atypical endometrial hyperplasia(AEH)and early endometrial carcinoma(EC)complicated with metabolic syndrome(obesity,insulin resistance and diabetes).Methods A total of 42 patients with AEH and early EC complicated with metabolic syndrome who were admitted to the Fifth Affiliated Hospital of Zhengzhou University from June 2014 to June 2019 were selected as the observation objects.According to the random number table method they were divided into control group(20 cases)and observation group(22 cases).The control group was treated with metformin combined with progesterone,while the observation group was treated with metformin combined with LNG-IUS.Before and after treatment the therapeutic effect and the rate of side effect were compared.Results The total effective rate was similar between the two groups(P>0.05).The rate of side effect in the observation group was lower than that in the control group(P<0.05).Conclusion Metformin combined with LNG-IUS is equivalent to metformin combined with progesterone in the treatment of AEH and early EC complicated with metabolic syndrome,but metformin combined with LNG-IUS has less side effects and low incidence of adverse reactions.
