超声造影联合剪切波弹性成像对甲状腺结节性质的评估分析

摘要 目的分析超声造影联合剪切波弹性成像(SWE)对甲状腺结节性质的评估价值。方法选择2018年1月至2021年7月洛阳新区人民医院接收的80例甲状腺结节患者为研究对象,患者入院时均接受超声造影及SWE检查,以病理组织活检结果为金标准,记录甲状腺结节性质,并比较恶性病变与良性病变甲状腺结节超声造影参数[上升时间(RT)、峰值强度(PI)及达峰时间(APT)]及SWE参数值[杨氏模量最大值(E_(max))、杨氏模量最小值(E_(mix))、杨氏模量平均值(E_(mean))],分析超声造影联合SWE对甲状腺结节性质的评估价值。结果病理组织活检结果显示,80例患者中恶性病变23例,良性病变57例;恶性病变甲状腺结节超声造影参数PI值小于良性病变甲状腺结节,差异有统计学意义(P<0.05);恶性病变甲状腺结节SWE参数E_(max)值大于良性病变甲状腺结节,差异有统计学意义(P<0.05);超声造影、SWE单独及联合检测评估甲状腺结节良恶性质的AUC分别为0.855、0.850、0.884,均有一定评估价值,且当PI、E_(max)分别取5.940 dB、69.965 kPa时,可获得最佳评估价值。结论超声造影联合SWE对甲状腺结节良恶性质具有较高评估价值。 Objective To analyze the value of contrast-enhanced ultrasonography combined with shear wave elastography(SWE)in the evaluation of the nature of thyroid nodules.Methods Eighty patients with thyroid nodules received by the hospital from January 2018 to July 2021 were selected as the research subjects,all patients underwent contrast-enhanced ultrasonography and SWE at admission,the results of pathological tissue biopsy were taken as the gold standard,recorded the nature of thyroid nodules,and the contrast-enhanced ultrasound parameters[rise time(RT),peak intensity(PI)and appear to peak time(APT)]and SWE parameter values[maximum elasticity of breast space occupying(E_(max)),minimum elasticity breast space occupying(E_(mix))and mean elasticity of breast space occupying(E_(mean))]were compared between malignant and benign thyroid nodules,the value of contrast-enhanced ultrasonography combined with SWE in the evaluation of the nature of thyroid nodules was analyzed.Results The results of pathological biopsy showed that among the 80 patients,23 were malignant lesions and 57 were benign lesions;the PI value of contrast-enhanced ultrasound parameters in malignant thyroid nodules was lower than that in benign thyroid nodules(P<0.05);the value of SWE parameter E_(max) in malignant thyroid nodules was higher than that in benign thyroid nodules(P<0.05);the AUC of contrast-enhanced ultrasonography and SWE in single and combined detection in evaluating benign and malignant nature of thyroid nodules were 0.855,0.850 and 0.884,respectively,which had certain appraisal value.When PI and E_(max) were taken as 5.940 dB and 69.965 kPa,respectively,the best evaluation value could be obtained.Conclusion Contrast-enhanced ultrasonography combined with SWE has higher value in evaluating the benign and malignant nature of thyroid nodules.
