摘要
目的分析超声心动图(UCG)联合组织多普勒成像(TDI)心肌做功指数(Tei指数)在先天性心脏病(CHD)患儿评估中的应用价值。方法选择2019年1月至2021年3月新郑市公立人民医院收治的82例疑似CHD患儿为研究对象,患儿入院后均接受二维UCG及TDI检查,以数字减影血管造影(DSA)结果作为CHD最终诊断标准并分组,分为CHD组及非CHD组,调查并记录患儿相关资料、UCG参数[左室舒张末内径(LVD)、二尖瓣舒张早期最大血流速度(E)/晚期最大血流速度(A)]及TDI-Tei指数,分析UCG联合TDI-Tei指数对CHD的评估价值。结果以DSA检查结果为金标准,82例疑似CHD患儿中59例确诊为CHD,占71.95%(59/82);CHD组患儿LVD、TDI-Tei指数值大于非CHD组患儿,E/A值小于非CHD组患儿,差异有统计学意义(P<0.05);LVD、E/A、TDI-Tei指数单独及联合检测评估CHD的曲线下面积(AUC)分别为0.775、0.808、0.822、0.837,均有一定评估价值,且当LVD、E/A、TDI-Tei指数分别取49.880 mm、1.455、0.525时,可获得最佳评估价值。结论UCG联合TDI-Tei指数在CHD患儿评估中具有较高应用价值。
Objective To analyze the application value of echocardiography(UCG)combined with tissue Doppler imaging(TDI)myocardial work index(Tei index)in the evaluation of children with congenital heart disease(CHD).Methods A total of 82 children with suspected CHD treated in Public People’s Hospital of Xinzheng from January 2019 to March 2021 were selected as the research subjects,all children underwent two-dimensional UCG and TDI after admission,the results of digital subtraction angiography(DSA)were taken as the final diagnostic criteria of CHD and divided into CHD group and non CHD group,the relevant data,UCG parameters[left ventricular end diastolic diameter(LVD),early mitral maximum flow velocity(E)/late maximum flow velocity(A)]and TDI Tei index of children were investigated and recorded,the evaluation value of UCG combined with TDI-Tei index for CHD was analyzed.Results DSA examination results were taken as the gold standard.Fifty-nine of the eighty-two children with suspected CHD were diagnosed as CHD,accounting for 71.95%(59/82).The values of LVD and TDI-Tei of children in CHD group were higher than those in non CHD group,and the E/A value was lower than that in non CHD group(P<0.05).The area under curve(AUC)of CHD evaluated by LVD,E/A and TDI-Tei indexes alone and jointly were 0.775,0.808,0.822 and 0.837,respectively,which had certain evaluation value.The best evaluation value can be obtained when LVD,E/A and TDI-Tei indexes were 49.880 mm,1.455 and 0.525,respectively.Conclusion UCG combined with TDI-Tei index has high application value in the evaluation of children with CHD.
