开闭链训练对肩胛骨动力异常人群上肢稳定性的影响 被引量:1

Effect of Open and Closed Kinetic Chain Exercises on Upper Limb Stability in Patients with Scapular Dyskinesis

摘要 目的:明确开链训练与闭链训练对肩胛骨动力异常人群上肢稳定性的影响,为肩胛骨动力异常康复和上肢稳定性训练提供新思路,指导临床实践。方法:将36名(男17,女19)肩胛骨动力异常受试者随机分为对照组、开链组和闭链组,对照组不进行干预,开链组采用开链训练进行干预,闭链组采用闭链训练进行干预,干预周期为6周,每周干预3次,干预前后分别对受试者进行Y-Balance测试和上肢闭链稳定性测试,对比测试结果变化,组内使用配对样本t检验,组间使用方差分析。结果:开链组干预后YBT和CKCUEST结果明显高于干预前结果(P<0.05);闭链组干预后YBT和CKCUEST结果明显高于干预前结果(P<0.01);闭链组YBT和CKCUEST结果明显高于开链组结果(P<0.05)。结论:闭链训练与开链训练均可以改善肩胛骨动力异常人群上肢稳定性,闭链训练比开链训练的训练效果更理想,在肩胛骨动力异常的康复中可多加入闭链训练,提高训练效果;肩胛骨肌群影响肩胛骨动力异常人群的上肢稳定性,训练中肩胛骨肌群和肩袖肌群需要同时考虑。 Objective:To clarify the effects of open chain training and closed chain training on upper limb stability in people with abnormal scapular dyskinesis,so as to provide new ideas for the rehabilitation of abnormal scapular dyskinesis and upper limb stability training,and guide clinical practice.Methods:36 subjects with abnormal scapular dyskinesis(male 17,female 19)were randomly divided into control group,open chain group and closed chain group.The control group did not intervene,the open chain group used open chain training for intervention,and the closed chain group used closed chain training for intervention.The intervention cycle was 6 weeks,3 times a week.Before and after the intervention,Y-Balance test and upper limb closed chain stability test were carried out respectively,comparing the changes of test results,paired sample t-test was used within the group,and analysis of variance was used between the groups.Results:The results of YBT and CKCUEST were significantly improved after training in the open chain group(P<0.05)and the closed chain group(P<0.01).After training,the results of YBT and CKCUEST in the closed chain group were significantly higher than the results in the open chain group.Conclusion:Both open and closed kinetic chain exercises can improve upper limb stability in patients with scapular dyskinesis,the effect of closed kinetic chain exercises is better than open kinetic chain exercises.In rehabilitation of scapular dyskinesis,trainer can use a lot of closed kinetic chain exercises to improve upper limb stability.Scapular muscles can affect upper limb stability in patients with scapular dyskinesis,therapist shoulder consider both scapular muscles and rotator cuff in the process of rehabilitation.
