肩关节镜下肱二头肌长头肌腱切断术治疗顽固性冻结肩的临床效果

Clinical efficacy of shoulder arthroscopic tenotomy of long head of biceps tendon for treatment of intractable frozen shoulder

目的探讨肩关节镜下肱二头肌长头肌腱(LHBT)切断术治疗顽固性冻结肩的临床效果。方法回顾性分析31例顽固性冻结肩患者的临床资料,患者均在肩关节镜下行LHBT切断术。分别于术前、术后6个月,采用疼痛视觉模拟量表(VAS)评分评估患者疼痛情况,采用美国肩肘外科医师协会(ASES)肩关节功能评分及Constant-Murley评分评估肩关节功能,并观察术后并发症的发生情况。结果所有患者术后均未出现感染,屈肘、前臂旋转功能均正常,仅有1例出现大力水手征,且无需特殊处理。术后6个月,患者的疼痛VAS评分低于术前,ASES肩关节功能评分和Constant-Murley评分均高于术前(均P<0.05)。结论采用肩关节镜下LHBT切断术治疗顽固性冻结肩患者效果确切,能有效地缓解患者肩部疼痛,促进肩关节功能恢复,且安全性高。

Objective To explore the clinical efficacy of shoulder arthroscopic tenotomy of the long head of biceps tendon(LHBT)for the treatment of intractable frozen shoulder.Methods The clinical data of 31 patients with intractable frozen shoulder were analyzed retrospectively.All patients underwent shoulder arthroscopic LHBT tenotomy.Before the operation and six months after the operation,the Visual Analogue Scale(VAS)pain score was used to evaluate patients′condition of pain,and the American Shoulder and Elbow Surgeons(ASES)score and Constant-Murley score to evaluate shoulder functions.Besides,the incidence of postoperative complications was observed.Results No infection occurred in any cases after the operation,and elbow flexion and rotation functions of forearm were normal,and only one case of Popeye sign appeared which did not need special treatment.Six months after the operation,VAS pain score was lower,ASES score for shoulder functions and Constant-Murley score were higher than those before the operation(all P<0.05).Conclusion Shoulder arthroscopic LHBT tenotomy has definite efficacy for the treatment of patients with intractable frozen shoulder,which can effectively relieve shoulder pain,promote the recovery of shoulder functions and achieve high safety in the patients.
