止痒灵坐浴对湿热下注型LSIL伴HR-HPV感染患者阴道微生态及HR-HPV转归的影响

Effect of Zhiyangling Sit Bath on Vaginal Microecology and HR-HPV Outcome of Patients with Damp-heat LSIL and HR-HPV Infection

目的观察止痒灵坐浴辅助治疗湿热下注型子宫颈低级别鳞状上皮内瘤变(LSIL)伴高危型人乳头状瘤病毒(HR-HPV)感染患者的效果,初步探讨中药止痒灵坐浴对调整阴道微生态方面可能存在的治疗作用,为临床运用中医药治疗LSIL提供参考。方法选取2019年3月至2020年3月在泉州市中医院中医妇科门诊就诊的HR-HPV感染患者,同时符合西医诊断标准及中医带下病湿热下注证诊断标准的LSIL患者共180例,其中试验组(采用止痒灵坐浴+保妇康栓阴道给药治疗3个月)90例,对照组(采用保妇康栓阴道给药治疗3个月)90例。比较治疗前后阴道微生态指标变化、中医症状积分及1年后复查HR-HPV转阴率。结果治疗后试验组患者的中医症状积分低于对照组和治疗前,差异有统计学意义(P<0.05)。治疗后试验组阴道微生态失调者均低于对照组和治疗前,差异有统计学意义(P<0.05)。试验组阴道微生态失调者经治疗后HR-HPV转阴率明显高于对照组,差异有统计学意义(P<0.05)。试验组阴道微生态正常者经治疗后HR-HPV转阴率与对照组比较,差异无统计学意义(P>0.05)。结论中医辩证运用止痒灵坐浴辅助治疗湿热下注型LSIL伴HR-HPV感染患者能明显缓解患者的临床症状,改善阴道微生态,有助于提高患者HR-HPV的转阴率。

Objective To observe the effect of Zhiyangling sitz bath in adjuvant treatment of low-grade cervical squamous intraepithelial lesions(LSIL)with high-risk human papillomavirus(HR-HPV)infection with damp-heat betting type,and to preliminarily explore the traditional Chinese medicine Zhiyangling sitz bath The possible therapeutic effects on the adjustment of vaginal microecology provide a reference for the clinical application of traditional Chinese medicine in the treatment of LSIL.Method A total of 180 patients with HR-HPV infection who attended the gynecological clinic of Quanzhou Hospital of Traditional Chinese Medicine from March 2019 to March 2020 and met the diagnostic criteria of Western medicine and the diagnostic criteria of dampheat syndrome of TCM disease were selected.Among them,180 LSIL patients 90 cases in the test group(treated with Zhiyangling sitting bath+Baofukang suppository vaginal administration for 3 months),and 90 cases in the control group(treated with Baofukang suppository vaginal administration for 3 months).The changes of vaginal microecological indicators and TCM symptom scores before and after treatment were compared,and the HR-HPV conversion rate was rechecked after 1 year.Result After treatment,the TCM symptom scores of patients in the test group were lower than those in the control group and before treatment,and the difference was statistically significant(P<0.05).After treatment,the vaginal microecological disorders in the test group were lower than those in the control group and before treatment,and the difference was statistically significant(P<0.05).The HR-HPV negative rate of vaginal microecological disorders in the experimental group was significantly higher than that in the control group after treatment,and the difference was statistically significant(P<0.05).Compared with the control group,the negative rate of HR-HPV in the test group with normal vaginal microecology was not statistically significant(P>0.05).Conclusion Chinese medicine dialectical use of Zhiyangling sitz bath to assist patients with damp-heat betting type LSIL with HR-HPV infection can significantly alleviate the clinical symptoms of the patients,improve the vaginal microecology,and help increase the rate of HR-HPV conversion.
