性治疗联合真空负压水动气动气泡按摩在临时性射精障碍中的应用研究

Application of sexual therapy combined with vacuum negative pressure hydro pneumatic/pneumatic bubble massage in male temporary ejaculation failure

目的探讨性治疗联合真空负压水动气动气泡按摩在临时性射精障碍中的临床应用效果。方法选取2019年1月至2021年1月南京大学医学院附属鼓楼医院诊治的85例拟行辅助生殖,主诉在家夫妻同房、手淫可射精,来院后手淫临时取精困难患者(非女方取卵日)作为研究对象。门诊给予国际勃起功能指数5(IIEF-5)评分调查患者的勃起情况。告知后续治疗方案,患者及家属自主选择后将其分为两组,A组(n=43)为性治疗联合真空负压水动气动气泡按摩组,B组(n=42)为来院取精当日口服西地那非组。1个月后统计两组手淫取精结果。结果 85例患者IIEF-5评分范围为22~25分,均无勃起功能障碍。来院取精当日,A组成功取精37例(86.05%),B组成功取精23例(54.76%),差异具有统计学意义(P<0.05)。结论相较于单纯临时口服西地那非,男性临时性射精障碍患者采用性治疗联合真空负压水动气动气泡按摩的方法可以有效提高手淫获精率。

Objective To investigate the clinical effect of sex therapy combined with vacuum negative pressure hydro pneumatic/pneumatic bubble massage in male temporary ejaculation failure. Methods From January 2019 to January 2021, 85 patients who planned to receive assisted fertility treatment in Drum Tower Hospital Affiliated to Medical College of Nanjing University were selected as the research subjects. The chief complaint was that they could ejaculate when having sex at home with wife or masturbation, while having temporarily difficulty in obtaining sperm in the hospital(not the date of female egg retrieval). The erectile status of the patients was investigated by the international index of erectile function-5(IIEF-5) score in the outpatient. The follow-up treatment plan was informed. The patients and their family members were divided into two groups after their independently selection: group A(n=43, sex therapy combined with vacuum negative pressure hydropneumatic bubble massage), and group B(n=42, sildenafil oral treatment on the day of coming to the hospital for extraction). After one month of treatment, the results of sperm extraction in the two groups were observed. Results IIEF-5 scores of 85 patients were 22-25 respectively, and none of them had erectile dysfunction. On the day of sperm collection, 37 cases(86.05%) in group A and 23 cases(54.76%) in group B were successfully collected, the difference was statistically significant(P<0.05). Conclusions Compared with temporary oral sildenafil alone, sex therapy combined with vacuum negative pressure hydropneumatic bubble massage can improve the rate of masturbation in male patients with temporary ejaculatory dysfunction more effectively.
