上消化道碘克沙醇造影在新生儿消化道畸形中的诊断价值及安全性

The Diagnostic Value and Safety of Upper Gastrointestinal Iodixanol Imaging in Neonatal Gastrointestinal Malformations

目的:探讨上消化道碘克沙醇造影在新生儿消化道畸形中的诊断价值及安全性。方法:研究对象选取我院2016年4月至2020年6月间收治的经手术确诊为消化道畸形新生儿49例,采用多次少量注入碘克沙醇造影剂的方法行上消化道造影检查。分析患儿上消化道造影阳性结果情况以及不良反应发生情况。结果:经上消化造影检查符合消化道畸形者29例,其中食道闭锁11人,十二指肠隔膜3人,幽门梗阻7人,中肠旋转不良6人,胃扭转2人,阳性率为59.18%;存在1例轻微恶心,其余患儿均未出现不良反应;所有患儿造影前后血清尿素氮、肌酐值比较,差异均无统计学意义(P>0.05)。结论:上消化道碘克沙醇造影应用于新生儿消化道畸形可弥补钡剂造影引起不良反应的缺点,具备一定的安全可行性。

Objective:To investigate the diagnostic value and safety of upper gastrointestinal iodixanol imaging in neonatal gastrointestinal malformations.Methods:The study subjects selected 49 neonates with gastrointestinal malformations who were admitted to our hospital from April 2016 to June 2020 and were diagnosed with gastrointestinal malformations. The upper gastrointestinal angiography was performed with multiple injections of iodixanol contrast agent. Analyze the positive results of upper gastrointestinal angiography and the occurrence of adverse reactions in children.Results:There were 29 patients with digestive tract malformations by upper gastrointestinal angiography, including 11 patients with esophageal atresia, 3 patients with duodenal septum, 7 patients with pyloric obstruction, 6 patients with midgut malrotation, and 2 patients with gastric volvulus. The positive rate was 59.18. %;There was 1 case of mild nausea, and the other children had no adverse reactions. There was no significant difference in serum urea nitrogen and creatinine values of all children before and after angiography(P>0.05).Conclusion:The application of iodixanol imaging of the upper gastrointestinal tract to neonatal gastrointestinal malformations can make up for the disadvantages of barium imaging caused by adverse reactions, and it is safe and feasible.
