持续性血液净化对重症脓毒症患者血流动力学的影响

Effect of continuous blood purification on the blood flow of patients with severe sepsis

目的探讨持续性血液净化(CBP)对重症脓毒症患者血流动力学的影响。方法选择2019年1月至2020年12月赫章县人民医院收治的重症脓毒症患者100例为研究对象,按治疗方法不同分为试验组与对照组,对照组采取常规治疗,试验组在对照组基础上加用CBP治疗,检测并比较2组治疗前后的血流动力学指标,以及2组治疗前后的血乳酸及炎症因子水平。结果试验组治疗后的血管阻力指数(SVRI)指标高于对照组(P<0.05),而2组的心脏指数(CI)、心率及平均动脉压(MAP)差异无统计学意义(P>0.05);试验组治疗后的血乳酸、肿瘤坏死因子(TNF)-α及C反应蛋白(CRP)均低于对照组(P<0.05)。结论对重症脓毒症患者采用CBP治疗,能够促进机体血流动力学指标的有效恢复,改善其血乳酸及炎症水平,值得临床推广应用。

Objective To investigate the effect of continuous blood purification(CBP)on the blood flow of patients with severe sepsis.Methods One hundred patients were selected as the research objects,who were admitted to Hezhang County People′s Hospital between January 2019 and December 2020 because of severe sepsis.The patients were equally divided into the experimental group and control group according to treatment method.The control group was treated with conventional treatment.The experimental group was treated with CBP on the basis of the control group.The patients of each group were checked and compared before and after the treatments in the following aspects:the hemodynamic indexs,the blood lactic acid and inflammatory factor levels.Results After treatment the SVRI index of the experimental group after treatment was higher than that of the control group(P<0.05),while the cardiac index(CI),heart rate(HR)and mean arterial pressure(MAP)indexes of the two groups were not significantly different(P<0.05);the blood lactic acid and tumor necrosis factor-α(TNF-α)of the experimental group and C-reactionprotein(CRP)indexes were lower than those of the control group(P<0.05).Conclusion CBP treatment for patients with severe sepsis can promote the effective recovery of body hemodynamic indicators and improve their blood lactic acid and inflammation levels.It is worthy of clinical application.
