MRS波谱评价常规磁共振阴性高热惊厥幼儿海马早期损伤的研究

Early Hippocampal Injury in Children with Conventional Magnetic Resonance Negative Hyperpyretic Convulsion Was Evaluated by Magnetic Resonance Spectroscopy of 3.0T

摘要 目的:运用3.0T波谱对常规磁共振阴性的高热惊厥幼儿海马早期损伤进行前瞻性研究。方法:共收集2017-2019年度3岁以下幼儿单纯性高热惊厥(SFC)组21例、复杂性高热惊厥(CFC)组12例、以及健康体检儿童6例(CON),采集各组双侧海马的1H-MRS数据,测量并计算实验组及对照组的代谢物比值NAA/Cr、Cho/Cr、NAA/Cho、NAA/Cr+Cho。使用单因素方差分析比较组间MRS代谢参数,使用配对t检验比较左右海马MRS代谢参数,使用Pearson相关分析探索患儿年龄、血乳酸水平与代谢参数之间的关系。结果:SFC、CFC、CON组间MRS代谢参数无统计学差异(P>0.05),幼儿左右海马代谢物参数无统计学差异(P>0.05)。0-3岁高热惊厥患儿年龄与双侧海马NAA/Cho+Cr显著正相关(相关系数左/右:0.42/0.41,P<0.05),年龄与左侧海马Cho/Cr显著负相关(相关系数:-0.44,P<0.05);血乳酸水平与MRS代谢参数未发现明显相关关系。结论:常规磁共振阴性高热惊厥幼儿海马MRS波谱未发现明显病理性损害。 Objective:The early hippocampal injury of children with high fever convulsion was prospectively studied by 3.0 T spectrum.Methods:a total of 21 cases of simple hyperpyretic convulsion(SFC) group, 12 cases of complicated hyperpyreticconvulsion(CFC) group and 6 cases of healthy children(CON) were collected in 2017-2019.1 H-MRS data of the bilateral hippocampus of each group were collected. One-way anova was used to compare MRS metabolic parameters between groups, paired t test was used to compare left and right MRS metabolic parameters in hippocampus, and Pearson correlation analysis was used to explore the relationship between age, blood lactic acid level and metabolic parameters in children.Results:there was no statistical difference in MRS metabolic parameters between SFC, CFC and CON groups(P>0.05), and no statistical difference in left and right hippocampal metabolite parameters in children(P>0.05). There was a significant positive correlation between age and bilateral hippocampal NAA/Cho+Cr(correlation coefficient left/right: 0.42/0.41, P<0.05), and a significant negative correlation between age and left hippocampal Cho/Cr(correlation coefficient-0.44, P<0.05). No significant correlation was found between blood lactic acid level and MRS metabolic parameters.Conclusion:no pathological damage was found in MRS spectrum of hippocampus in children with conventional magnetic resonance negative hyperpyretic convulsion.
