盆神经节在前列腺炎导致排尿功能障碍中的作用及其机制

摘要 目的探讨盆神经节在前列腺炎导致排尿功能障碍中的作用及其机制。方法将22只雄性SD大鼠随机数字法分为两组:前列腺炎模型组(n=10)、对照组(n=12)。通过前列腺内注射1%无菌角叉菜胶溶液建立大鼠的前列腺炎模型,检测前列腺炎模型组和对照组的尿动力学指标的变化。利多卡因阻滞盆神经节后检测前列腺炎模型大鼠的尿动力学参数。采用膀胱壁内注射荧光金(FG)前列腺内注射快蓝(FB)行逆行神经追踪双标技术检测盆神经节内是否存在同时支配膀胱和前列腺的神经元。组间比较采用单因素方差分析。结果盆神经节切片内科观察到FG-FB双标神经元,证实盆神经节内存在同时支配膀胱和前列腺的神经元。大鼠的尿动力学结果显示,前列腺炎组阻滞前膀胱容量(1.01±0.03)ml大于正常组膀胱容量(1.00±0.02)ml及前列腺炎组阻滞后膀胱容量(1.00±0.02)ml,差异无统计学意义(F=0.318,P>0.05),前列腺炎组阻滞后大鼠排尿频率(12.6±0.13)次/小时低于前列腺炎组阻滞前大鼠排尿频率(24.55±2.14)次/小时,差异有统计学意义(F=502.622,P<0.05)。前列腺炎组阻滞后大鼠不稳定收缩次数(1.5±0.5)次/小时少于前列腺炎组阻滞前大鼠不稳定收缩次数(22.3±2.6)次/小时,差异有统计学意义(F=160.175,P<0.05)。结论前列腺内注射无菌角叉菜胶溶液可成功建立前列腺炎大鼠模型。盆神经节内存在同时支配膀胱和前列腺的神经元,盆神经节在前列腺炎导致的排尿功能障碍中起重要的调节作用。 Objective To explore the role and mechanism of pelvic ganglia in urinary dysfunction caused by prostatitis.Methods Twenty-two male Sprague Dawley(SD)rats were divided into two groups by random numbers:the prostatitis model group(n=10)and the control group(n=12).The rat model of prostatitis was established by injecting 1%sterile carrageenan solution into the prostate,and the changes of urodynamic indexes in the prostatitis model group and the control group were detected.After lidocaine block the pelvic ganglion,the urodynamic parameters of prostatitis model rats were measured.Fluorescent gold(FG)was injected into the bladder wall and fast blue(FB)was injected into the prostate were used to detect whether there are neurons in the pelvic ganglia that simultaneously innervate the bladder and prostate.SPSS 22.0 was used for statistical analysis,and the comparison between groups was performed by one-way analysis of variance.Results FG-FB double-labeled neurons were observed in the pelvic ganglion section,which confirmed that there were neurons that innervated the bladder and prostate at the same time in the pelvic ganglia.The urodynamic results of the rats showed that The bladder volume before block in the prostatitis group[(1.01±0.03)ml]was larger than that in the normal group[(1.00±0.02)ml]and the bladder volume after block in the prostatitis group[(1.00±0.02)ml],the difference was not statistically significant(F=0.318,P>0.05),the urination frequency of rats in the prostatitis group after blockade was(12.6±0.13)times/hour was lower than that of the rats in the prostatitis group before blockade[(24.55±2.14)times/hour],the difference was statistically significant(F=502.622,P<0.05).The number of unstable contractions of rats in the prostatitis group after blockade was(1.5±0.5)times/hour was less than that of rats in the prostatitis group before blockade[(22.3±2.6)times/hour],the difference was statistically significant(F=160.175,P<0.05).Conclusion Injecting sterile carrageenan solution into the prostate can successfully establish a rat model of prostatitis.There are neurons in the pelvic ganglia that simultaneously innervate the bladder and prostate.The pelvic ganglia play an important role in regulating urinary dysfunction caused by prostatitis.
