脑胶质瘤切除术中肿瘤切缘定位中B超的应用价值

Application of B ultrasound in tumor margin localization during glioma resection

目的探讨在脑胶质瘤患者的切除手术中B超在肿瘤切缘定位中的作用。方法将内蒙古自治区人民医院2018年1月至2019年1月收治的66例脑胶质瘤患者作为研究对象,按照不同的检查方式分为实验组(n=33)与对照组(n=33),将两组患者术后肿瘤的残留率进行对比。结果对照组患者术后残留5(15.15%)例,实验组术后残留2(6.06%)例,实验组的术后残留率明显低于对照组(χ^(2)=5.345,P<0.05)。实验组治疗后2、7 d颅内压[(3.43±0.42)、(2.83±0.42)kPa]明显低于对照组[(4.03±0.37)、(3.69±0.28)kPa,t=12.683、16.810,P<0.05]。实验组高级别胶质瘤患者术后评分高于对照组,实验组注意力(106.54±6.56)、执行能力(105.76±6.77)、抽象及定向评分(106.65±6.57)显著高于对照组,差异有统计学意义(t=4.130、3.535、5.680,P<0.05)。治疗后实验组注意力、执行能力、抽象及定向评分显著高于对照组,差异有统计学意义(t=9.355、9.606、10.575,P<0.05)。结论在脑胶质瘤患者的肿瘤切除过程中应用B超对患者进行肿瘤切缘定位可以准确定位患者肿瘤位置、肿瘤范围,减少患者肿瘤的残留率,提高疗效。

Objective To investigate the role of B-mode ultrasound in the localization of tumor resection margins in patients with glioma.Methods A total of 66 patients with glioma admitted to our hospital from January 2018 to January 2019 were selected as study subjects.According to different examination methods,they were divided into the experimental group(n=33)and the control group(n=33),and the postoperative tumor residual rate of the two groups of patients was compared.SPSS19.0 software was calculated,count data[n(%)]indicated,using test.Meometric data(s)are indicated by t-test.Results 5(15.15%)and 2(6.06%)in the control group were significantly lower than controls(χ^(2)=5.345,P<0.05).intracranial BP[(2.43±0.42),(2.83±0.42)kPa]]was significantly lower than control group[(4.03±0.37),(3.69±0.28)kPa,t=12.683,16.810,P<0.05].Patients with high-grade glioma had higher postoperative scores than controls,and attention(106.54±6.56),executive ability(105.76±6.77),abstract and orientation scores(106.65±6.57)were significantly higher than statistically significant(t=4.130,3.535,5.680,P<0.05).After treatment experimental attention,executive ability,abstraction and orientation scores were significantly higher than the control group,statistically significant(t=9.355,9.606,10.575,P<0.05).Conclusion B-ultrasonography can accurately locate the tumor location and tumor scope in patients with glioma during tumor resection,reduce the residual rate of the tumor and improve the curative effect.
