当归芍药散联合运动疗法治疗非酒精性单纯性脂肪肝肝郁脾虚证疗效观察

Clinical effect of Danggui Shaoyao Powder combined with exercise therapy for nonalcoholic simple fatty liver of liver-depression spleen-deficiency

目的观察当归芍药散联合运动疗法治疗非酒精性单纯性脂肪肝(NAFL)肝郁脾虚证的临床疗效。方法将80例NAFL肝郁脾虚证患者按照随机数字表法分为2组。对照组40例行运动疗法治疗,治疗组40例在对照组治疗基础上加当归芍药散口服治疗。2组疗程均为12周。比较2组治疗前后肝功能指标丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、γ-谷氨酰转移酶(GGT),血脂指标甘油三酯(TG)、总胆固醇(TC)水平,肝脂肪受控衰减参数(CAP)、体质量指数(BMI)变化。统计比较2组中医证候疗效及实验室指标疗效。结果治疗后2组ALT、AST、GGT、TG、TC水平及肝脏CAP值、BMI均较本组治疗前降低(P<0.05),且治疗组均低于对照组(P<0.05)。治疗后2组中医证候各项评分均较本组治疗前降低(P<0.05),且治疗组均低于对照组(P<0.05)。治疗组中医证候总有效率为60%(24/40),实验室指标总有效率为37.5%(15/40),对照组分别为35%(14/40)、25%(10/40),治疗组中医证候疗效及实验室指标疗效均优于对照组(P<0.05)。结论当归芍药散联合运动疗法治疗NAFL肝郁脾虚证具有一定临床疗效,值得临床推广运用。

Objective To observe the clinical effect of Danggui Shaoyao Powder combined with exercise therapy for nonalcoholic simple fatty liver(NAFL)of liver-depression spleen-deficiency.Methods Totally 80 patients with NAFL(liver-depression spleen-deficiency)were randomly divided into two groups,40 cases in control group received exercise therapy,and 40 cases in treatment group additionally received Danggui Shaoyao Powder on the basis of control group.Treated for 12 weeks,the aim was to compare liver function indexes including alanine aminotransferase(ALT),aspartate aminotransferase(AST),gamma-glutamyltransferase(GGT);blood lipid indexes including triglyceride(TG),total cholesterol(TC);controlled attenuation parameters(CAP)of liver fats,and body mass index(BMI);the curative effect of traditional Chinese medicine(TCM)syndrome and laboratory index was counted.Results After treatment,level of ALT,AST,GGT,TG,TC,CAP,BMI in groups were decreased(P<0.05),which decreased notably in treatment group when compared with control group(P<0.05);TCM symptom scores in groups remarkably decreased(P<0.05),improvement of which in treatment group were significantly better than that in control group(P<0.05).The total effective rates of TCM syndrome in treatment group and control group were 60.0%(24/40)and 35.0%(14/40),respectively,the curative effect being better in treatment group than control group(P<0.05);The total effective rates of laboratory index were better in treatment group than in control group[37.5%(15/40)vs 25.0%(10/40),(P<0.05),respectively].Conclusion For patients with NAFL(liver-depression spleen-deficiency),Danggui Shaoyao Powder combined with exercise therapy has a certain clinical effect with worthy of clinical application.
