基于结构方程模型的心力衰竭患者住院费用影响因素分析

Analysis on the influence factor of hospitalization costs of patients with heart failure based on structural equation model

[目的]分析影响心力衰竭患者住院费用的相关因素,为减轻心衰患者疾病负担、合理控制住院费用提供参考。[方法]收集样本医院2016-2018年病案首页数据库中疾病主要诊断为心力衰竭(I50)的患者678例。采用单因素分析和结构方程模型分析心衰患者住院费用的影响因素。[结果]心衰患者住院费用年均增长率为-2.68%,住院总费用控制较好,药品费和检查检验费占比较大。[结论]心衰患者住院费用相对较高,住院天数对住院费用的影响最大,住院天数是控制住院费用的关键因素;性别、年龄、手术、合并症个数、抢救、医院感染既直接影响住院费用,也通过住院天数间接影响住院费用。

Objective To analyze the related factors affecting hospitalization cost of patients with heart failure so as to provide reference for reducing disease burden of patients with heart failure and reasonably control hospitalization cost.Methods It collected 678 heart failure patients in sample hospitals whom recorded in on home page of medical record database and were diagnosed with heart failure(I50)from 2016 to 2018.The influencing factors of hospitalization cost of heart failure patients were analyzed by single factor analysis and structural equation model.Results The average annual growth rate of patients with heart failure hospitalization cost was-2.68%,the overall hospitalization cost controlled well,drug cost and examination cost accounted for a large proportion.Conclusions The hospitalization cost of patients with heart failure is high,and the length of stay has the biggest influence for hospitalization cost and which is the key factor of controlling hospitalization cost.Gender,age,surgery,number of complication,salvage,hospital infection directly influence hospitalization cost,and which indirectly influence hospitalization cost through length of stay as well.
